灰阶模式超微血管成像技术评估甲状腺结节良恶性的应用价值

摘  要: 目的探讨灰阶模式超微血管成像(mSMI)技术在鉴别甲状腺结节良恶性中的应用价值.方法回顾性分析用超声检出并获得病理结果的120个甲状腺结节,分成良性组(48个甲状腺结节)、恶性组(72个甲状腺结节),对2组甲状腺结节分别行彩色多普勒血流显像(CDFI)和mSMI检查.利用Adler血流分级及甲状腺结节血流形态分型,比较CDFI和mSMI 2种技术对同一结节的血流检测能力的差异.结果mSMI对甲状腺结节内血管检出能力优于CDFI(P<0.001);mSMI与CDFI比较,mSMI能更好地还原甲状腺结节内血管的走行(P<0.001).结论mSMI与CDFI相比,mSMI能更好地显示甲状腺结节的血流分布及血流特征,对甲状腺结节良恶性鉴别诊断的灵敏度和特异度更高,mSMI为甲状腺结节良恶性的鉴别诊断提供新的方法. Objective To explore the application value of monochrome superb microvascular imaging(mSMI)in differential diagnosis of benign and malignant thyroid nodules.Methods 120 thyroid nodules detected by ultrasound and obtained pathological findings were analyzed retrospectively,which were divided into benign group(48 thyroid nodules),malignant group(72 thyroid nodules).Color Doppler flow imaging(CDFI)and mSMI examination were performed on two groups of thyroid nodules.Using Adler blood flow grading and thyroid nodule blood flow morphometry,the difference in blood flow detection ability between CDF1 and mSMI technigues were compared for the same nodule.Results mSMI was superior to CDFI in detecting vascularity in thyroid nodules(P<0.001).Compared with CDFI,mSMI could better show the shape of blood vessels in thyroid nodules(P<0.001).Conclusion Compared with CDFI,mSMI can better display the blood flow distribution and blood flow characteristics of thyroid nodules,and has higher sensitivity and secificity for diflrerential diagnosis of benign and malignant thyroid nodules.mSMI provides a new method for the differential diagnosis of benign and malignant thyroid nodules.
