摘 要: 目的动态观察宝鸡地区8例新型冠状病毒肺炎(COVID-19)患者的影像学特点,探讨COVID-19影像学表现在基层医院的临床应用价值。方法收集宝鸡地区2020年1月28日至2020年2月13日确诊的8例COVID-19病例,动态观察患者的胸部CT影像学特征,分析其影像学特点及与临床表现的相关性。结果发热后首次CT检查显示,2例患者胸部CT检查呈阴性,6例患者呈阳性,均表现为胸膜下多发病变,5例患者出现毛玻璃密度影(GGO),其中4例为实变影伴GGO,1例为纯GGO伴小血管增多增粗改变;发热后第2~3次复查发现病变数目增多,部分范围增大,以实变影为主,部分密度增高伴网格影;其中4例表现为部分病灶边缘弧形凹陷样收缩改变,肺结构扭曲现象;2例出现胸膜下弧形线样影改变。临床治愈末次复查CT发现病变出现不同程度的吸收缩小及索条影。结论COVID-19影像学表现具有一定特点,通过胸部CT影像学表现可以较早发现病变,对临床医生判断该病的预后和转归具有一定指导意义。 Objective To dynamically observe the imaging features of 8 confirmed cases of corona virus disease 2019(COVID-19)in Baoji area,and to investigate the clinical application value of CT imaging of COVID-19 in primary hospitals.Methods The 8 confirmed cases of COVID-19 in Baoji from January 28,2020 to February 13,2020 were collected,chest CT features of patients were dynamically observed,the characteristics of imaging manifestations of patients and its correlation with clinical manifestations were analyzed.Results The first CT examination after fever showed that 2 patients were negative in chest CT examination and 6 patients were positive,all of which showed subpleural multiple lesions,ground glass opacity(GGO)was found in 5 patients,of which 4 cases were solid change with GGO,1 case was pure GGO with increased small blood vessels.In the second to third reexamination after fever,it was found that the number of lesions increased,some areas increased,mainly solid shadows,some density increased with grid shadows;in which 4 cases showed arcuate concave contraction of the edge of some lesions and distorted lung structure;2 cases showed the changes of subpleural arc-shaped shadow.CT at the last review of clinical cure revealed that the lesions showed different degrees of absorption and shrinkage and cable shadow.Conclusion The imaging features of the COVID-19 have certain characteristics,and the lesions can be detected earlier by the chest CT imaging findings,which have certain guiding significance for clinicians to judge the prognosis and outcome of the disease.
