摘 要: 目的考察服用齐拉西酮门诊患者的血药浓度及其影响因素,为临床合理用药提供参考。方法回顾性分析2014年4月-2019年4月我院门诊患者的性别、年龄、剂量、合并用药对齐拉西酮血药浓度监测结果及浓度/剂量比(C/D)的影响。结果590例门诊患者的齐拉西酮平均血药质量浓度为(128.39±126.92)ng/mL,C/D为(1.53±1.41)(ng·d)/(mL·mg)。多元线性回归方程中,性别、年龄、给药剂量及合并用药只能解释16.10%的齐拉西酮血药浓度的变化。女性组齐拉西酮的C/D高于男性组(P<0.05);未成年组的C/D高于成年组(P<0.05),老年组的C/D与成年组、未成年组的差异无统计学意义(P>0.05);合并抗精神病药组的C/D高于无合并用药组(P<0.05),合并其他精神科药物组及多药联用组的C/D与无合并用药组的差异无统计学意义(P>0.05)。结论齐拉西酮血药浓度个体间差异大、影响因素复杂,合并抗精神病药物可能会引起齐拉西酮血药浓度升高,临床应加强监测血药浓度,减少合并用药,保障临床合理安全用药。 Objective To study the influencing factors of ziprasidone blood concentration in outpatients,and provide a reference for clinical rational drug use.Methods Retrospective analysis was used to study the influence of gender,age,dosage and combination of drugs on the monitoring results of ziprasidone concentration in blood and the concentration/dose(C/D)ratio of outpatients in our hospital from April 2014 to April 2019.Results The blood concentration of ziprasidone was(128.39±126.92)ng/mL and C/D ratio was(1.53±1.41)(ng·d)/(mL·mg)in 590 outpatients.Multiple linear regression showed that gender,age,dosage and drug combination only explained 16.10%of the changes in ziprasidone concentration.The C/D ratio of ziprasidone in female group was higher than that in male group(P<0.05).The C/D ratio of the juvenile group was higher than that of the adult group(P<0.05).There was no significant difference in C/D ratio between the elderly group and the juvenile group and the adult group(P>0.05).The C/D ratio of the group with antipsychotics was higher than that of the monotherapy group(P<0.05).The C/D ratio of the groups with other psychiatric drugs and with multi-drugs showed no significant difference from that of the monotherapy group.Conclusion The concentration of ziprasidone varies greatly among individuals,and the influencing factors are complex.The combination of antipsychotics may cause the concentration of ziprasidone to rise.In clinical practice,we should strengthen the monitoring of the concentration of ziprasidone,reduce the combination of drugs,and ensure the rational and safe use of drugs.
