摘　　要： 目的观察椎体成形术(PVP)治疗骨质疏松性压缩骨折引发腰背疼痛的效果。方法骨质疏松性压缩骨折患者90例,随机分为对照组(n=44)和观察组(n=46)。观察组以PVP治疗,对照组以常规开放性手术(经皮椎弓根穿刺和骨水泥注入手术)治疗,所有患者均以抗骨质疏松药物治疗,比较两组的住院时间、腰背疼痛缓解时间、下床时间、手术时间等临床指标。随访半年,比较两组发生神经损伤和感染、肺栓塞、骨水泥毒性反应、骨水泥外漏等并发症发生情况。结果观察组的住院时间、腰背疼痛缓解时间、下床时间、手术时间等临床指标显著少于对照组(P<0.01)。观察组并发症(神经损伤和感染、肺栓塞、骨水泥毒性反应、骨水泥外漏)的发生情况显著少于对照组(P<0.01)。结论用PVP治疗老年骨质疏松压缩性骨折引发的腰背疼痛,操作简便,患者容易耐受,效果较佳,术后并发症少。 Objective To observe the clinical efficacy of percutaneous vertebroplasty(PVP)in the treatment of lumbago and back pain induced by osteoporotic vertebral compression fr-ctures(OVCF).Methods 90 OVCF patients were selected and randomly divided into Control Group(n=44)and Observation Group(n=46).Observation Group was treated with PVP while Control Group was treated with conventional open operation(percutaneous pedicle puncture and bone cement injection).All the patients received the treatment with anti-osteoporosis drugs.The clinical indicators of the two groups were compared,such as hospitalization time,remission time of lumbago and back pain,ambulant time and operation duration.After 6 months of follow up,the incidence of such complications as nerve injury and infection,pulmonary embolism,as well as toxic reaction and leakage of bone cement in the two groups was compared.Results The hospitalization time,remission time of lumbago and back pain,ambulant time and operation duration in the Observation Group were significantly less than those of the Control Group(P<0.01);and the incidence rate of complications(nerve injury and infection,pulmonary embolism,as well as toxic reaction and leakage of bone cement)in the Observation Group was significantly lower than that in the Control Group(P<0.01).Conclusion Using PVP to treat lumbago and back pain induced by osteoporotic vertebral compression fractures is an easy operation,is tolerable for patients,has a optimal effect and has reduced postoperative complications.
