摘　　要： 目的验收经过多次PDCA循环干预后Ⅰ类切口手术抗菌药物预防使用管理的成效。方法跟踪2015—2018各年度1—6月份全院Ⅰ类切口抗菌药物预防使用情况。以2015年上半年未采取PDCA干预Ⅰ类切口手术957例为对照组,2016、2017和2018年上半年先后实施3轮PDCA管理Ⅰ类切口手术(分别为1 231、1 393和1 440例)为实验组。结果外科、乳腺外科及全院Ⅰ类切口平均预防性抗菌药物使用比例随PDCA管理的推进持续降低,分别从干预前的(45.41±16.87)%、(31.95±7.60)%和(36.83±6.25)%下降至(32.24±9.80)%、(30.25±7.15)%和(33.40±6.56)%(2016年第一轮干预),(23.41±6.34)%、(9.25±4.12)%和(18.77±4.70)%(2017年第二轮干预),最终达到(10.25±4.65)%、(7.55±1.81)%及(9.32±1.92)%(2018年第3轮干预)。结论PDCA循环可有效推动Ⅰ类切口抗生素预防使用比例的降低,为进一步提高抗菌药物管理水平以及加强临床合理用药提供了参考价值。 Objective To evaluate the effect of multiple PDCA cycles intervention on antibiotics preventive application in typeⅠincision.Methods The preventive application of antibiotics on typeⅠincision in our hospital between January and June from 2015 to 2018 were investigated.957 cases of typeⅠincision operation in the first half of 2015,without PDCA intervention,were served as the control group.1 231,1 393 and 1 440 cases of typeⅠincision operation in the first half of 2016,2017 and 2018 respectively,with three rounds of PDCA management,were served as experimental groups.Results The average proportions of antibiotics preventive application on typeⅠincision in surgery department,breast surgery department and the whole hospital were continuously decreased accompanied by the management of PDCA cycles from(45.41±16.87)%,(31.95±7.60)%and(36.83±6.25)%(2015;before PDCA intervention)to(32.24±9.80)%,(30.25±7.15)%and(33.40±6.56)%(2016;the first round of PDCA cycle),further to(23.41±6.34)%,(9.25±4.12)%and(18.77±4.70)%(2017;the second round of PDCA cycle),and finally to(10.25±4.65)%,(7.55±1.81)%,and(9.32±1.92)%(2018;the third round of PDCA cycle),respectively.Conclusion The PDCA cycles show a positive effect on the decline in proportion of antibiotics preventive application in typeⅠincision,which provide a great value in further improving the management of antibiotics and promoting clinical rational use of drugs.
