摘　　要： 目的 分析剖宫产孕妇利用罗哌卡因腰-硬联合麻醉的麻醉质量及对血流动力学的影响.方法 回顾性分析98例利用罗哌卡因腰-硬联合麻醉的剖宫产孕妇的临床资料,根据罗哌卡因的用量进行分组;其中对照组占45.92％（45/98）,使用1.5ml 1％罗哌卡因;观察组占54.08％（53/98）,使用1.0 ml 1％罗哌卡因;对比两组不同时间点心率、平均动脉压,以起效时间、运动阻滞时间、麻醉平面恢复时间、肌松满意度作为麻醉质量的观察指标.结果 观察组胎儿娩出后心率慢于对照组,平均动脉压高于对照组,差异有统计学意义（均P＜ 0.05）;两组其他时间点心率、平均动脉压、起效时间、运动阻滞时间、感觉阻滞时间、肌松满意度对比,差异均无统计学意义（均P＞ 0.05）.结论 剖宫产孕妇利用小剂量罗哌卡因腰-硬联合麻醉的麻醉质量较好,可减小对血流动力学的影响,值得临床研究使用. Objective To analyze the quality of anesthesia and hemodynamics of combined spinalepidural anesthesia with ropivacaine for pregnant women taking cesarean section.Methods The clinical data of 98 pregnant women taking spinal and epidural anesthesia with ropivacaine for cesarean section were retrospectively analyzed.The women were divided into a control group and an observation group according to the dosage of ropivacaine.The control group accounted for 45.92% （45/98）,and used 1.5 ml 1% ropivacaine;and the observation group accounted for 54.08% （53/98）,and used 1.0 ml 1% ropivacaine.The heart rate and mean arterial pressure at two different time points were compared between these two groups.The onset time,motor block time,recovery time of anesthesia plane,and satisfaction of muscle relaxation were used as the observation indicators of anesthesia quality.Results In the observation group,the heart rate after delivery of the fetus was slower and the mean arterial pressure was higher than those in the control group （P〈0.05）.There were no statistical differences in heart rate,mean arterial pressure,onset time,motor block time,sensory block time,and muscle relaxation satisfaction at other time points between the two groups （all P 〉 0.05）.Conclusions Combined spinal-epidural anesthesia with a low dose of ropivacaine for cesarean section pregnant women has better anesthesia quality and can reduce the impact on hemodynamics,so it is worth for clinical study.
