外周血T淋巴细胞与NTM肺病发病类型的相关性
The relative study between peripheral T lymphocyte and NTM lung disease

摘  要: 目的:探讨机体细胞免疫状态是否与非结核分枝杆菌(nontuberculosis mycobacteria,NTM)肺病的发病类型相关。方法:收集广州市胸科医院2013年7月至2015年6月收治的137例NTM肺病患者,根据痰或支冲液分枝杆菌培养结果,分为SGM肺病患者65例,RGM肺病患者72例,分析两组的T淋巴细胞亚群数值。结果:SGM组CD3^+T淋巴细胞(1133.32±311.72)×10~9/L;CD4^+T淋巴细胞(577.47±171.33)×10~9/L;CD8^+T淋巴细胞(298.27±87.06)×10~9/L;CD4^+/CD8^+T淋巴细胞比值2.02±0.50。RGM组CD3^+T淋巴细胞(603.14±249.86)×10~9/L;CD4^+T淋巴细胞(303.10±113.51)×10~9/L;CD8^+T淋巴细胞(247.98±156.50)×10~9/L;CD4^+/CD8^+T淋巴细胞比值1.26±0.52。P均<0.05,差异有统计学意义。结论:在NTM肺病中,RGM肺病的T淋巴细胞下降水平更明显,临床症状重,肺部空洞、支扩病灶多。 Objective:To discuss the correlation between cellular immunological status and nontuberculosis mycobateria (NTM) lung disease type. Methods: 137 NTM lung disease patients in Guangzhou Chest Hosptal from July 2013 to June 2015 were selected. According to sputum and bronchial lavage fluid tuberculosis culture result,patients were divided into slowly growing mycobateria (SGM) group (65 cases) and rapidly growing mycobateria (RGM) group (72 cases). Comparative study of T lymphocytes subgroup amounts between 2 groups was performed. Results : In SGM group, CD3 ^ T lymphocytes counting was ( 1133.32 ^ 311.72 ) ×10^9/L, CD4+ T lymphocytes ( 577.47 ± 171.33 )×10^9/L, CD8+ T lymphocytes ( 298.27 ± 87.06 )×10^9/L, and CD4+/CD8+ T lymphocytes ratio was 2.02±0.50. Whereas CD3+ T lymphocytes counting was ( 603.14± 249.86)×10^9/L ; CD4+ T lymphocytes( 303. 10± 113.51 ) ×10^9/L; CD8+T lymphoeytes (247.98± 156. 50) ×10^9/L; CD4+/CD8+ T lymphocytes ratio was 1.26±0.52 in RGM group ( P〈0.05).Conclusion : In NTM lung disease, RGM lung disease presented significantly decreased T lymphocyte level, clinical aggravation, and increased lung lesions.
