摘　　要： 目的观察超声引导下腹股沟韧带上髂筋膜阻滞在股骨PFNA内固定术中的应用效果。方法 60例股骨PFNA内固定术患者随机分为两组各30例,对照组在超声引导下行腹股沟韧带下髂筋膜阻滞麻醉,观察组在超声引导下行腹股沟韧带上髂筋膜阻滞麻醉。比较两组的术中麻醉药物应用情况、术后VAS评分以及相关指标。结果观察组患者的术中舒芬太尼、丙泊酚、瑞芬太尼用量显著低于对照组(P <0.05),而两组的手术时间无明显差异(P>0.05);观察组术后3 h、 6 h、 12 h、 24 h的疼痛VAS评分均显著低于对照组(P <0.05);观察组的苏醒时间、人工气道拔出时间、拔出30 min后Ramsay镇静评分、补救镇痛次数及血管活性药物使用次数均显著低于对照组(P <0.05),两组的出恢复室Ramsay镇静评分无明显差异(P>0.05)。结论超声引导下腹股沟韧带上髂筋膜阻滞在PFNA术中具有较好的应用效果,能够明显减少术中麻醉药物用量,改善术后镇痛效果。 Objective To observe the application effect of ultrasound-guided iliac fascia block on the inguinal ligament in PFNA internal fixation of femur.Methods 60 patients with PFNA internal fixation of femur were randomly divided into two groups,with 30 cases in each group.The control group underwent ultrasound-guided iliac fascia block under inguinal ligament,and the observation group underwent ultrasound-guided iliac fascia block on the inguinal ligament.The intraoperative use of anesthetics,postoperative VAS score and related indicators were compared between the two groups.Results The intraoperative dosage of sufentanil,propofol and remifentanil in the observation group were significantly lower than those in the control group(P<0.05),but no significant difference was found in the operation time between the two groups(P>0.05).The VAS scores of pain at 3,6,12 and 24 hours after operation in the observation group were lower than those in the control group(P<0.05).The recovery time,the time of artificial airway extraction,the Ramsay sedation score 30 minutes after extraction,the times of rescue analgesia and the times of using vasoactive drugs in the observation group were lower than those in the control group(P<0.05);No significant difference was found in Ramsay sedation score out of recovery room between the two groups(P>0.05).Conclusions Ultrasound-guided iliac fascia block on the inguinal ligament has good application effect in PFNA internal fixation of femur,which can significantly reduce the intraoperative dosage of anesthetics and improve the analgesic effect after operation.
