摘 要: 目的:分析手足口病并脑炎患儿S100β、脑脊液及脑电图检查的临床特点,以便提高重症手足口病的诊断率,为今后手足口病并脑炎患儿提供有力的诊断参考依据。方法:选取2017年1月-2018年8月本院收治的手足口病住院患儿405例。根据是否合并脑炎将其分为普通组(手足口病)304例和重症组(手足口病合并脑炎)101例。均进行S100β、脑电图检查,普通组中脑电图检查异常患儿和重症组进行脑脊液检查,比较两组检查结果,分析各指标的相关性。结果:普通组血清S100β水平低于重症组(P<0.05);普通组脑电图轻、中、重度异常率均明显低于重症组(P<0.05);普通组脑电图异常患儿脑脊液白细胞计数、蛋白质定量水平均低于重症组(P<0.05);S100β水平、脑电图异常率与脑脊液白细胞、蛋白质定量均呈正相关(P<0.05),S100β水平、脑电图异常率、脑脊液白细胞、蛋白质定量和病情的严重程度均呈正相关(P<0.05)。结论:血清S100β、脑脊液及脑电图检查在手足口病并脑炎患儿的诊断中具有较高临床意义,且S100β、脑电图可作为筛查重症手足口病的重要指标。 Objective:To analyze the clinical characteristics of S100β,cerebrospinal fluid and electroencephalogram in children with hand-foot-mouth disease and encephalitis,so as to improve the diagnostic rate of severe hand-foot-mouth disease,and to provide a strong diagnostic reference for children with hand-foot-mouth disease and encephalitis in the future.Method:A total of 405 children with hand-foot-mouth disease hospitalized in our hospital from January 2017 to August 2018 were selected.According to whether or not encephalitis was complicated,they were divided into normal group(hand-foot-mouth disease)304 cases and severe group(hand-foot-mouth disease combined with encephalitis)101 cases.S100βand electroencephalogram were performed in both groups,cerebrospinal fluid examination was performed in children with electroencephalogram abnormal results in normal group and severe group,the results were compared between two groups,and the correlation of each index was analyzed.Result:The level of serum S100βin normal group was lower than that of severe group(P<0.05).The rate of mild,moderate and severe abnormal electroencephalogram in normal group were significantly lower than those of severe group(P<0.05).The leucocyte count and protein quantitative level of children with abnormal electroencephalogram in normal group were lower than those of severe group(P<0.05).The levels of S100βand abnormal rate of electroencephalogram were positively correlated with leucocyte count and protein quantitative(P<0.05),the levels of S100β,abnormal rate of electroencephalogram,leucocyte count and protein quantitative were positively correlated with the severity of the disease(P<0.05).Conclusion:Serum S100β,cerebrospinal fluid and electroencephalogram have high clinical significance in the diagnosis of hand-foot-mouth disease and encephalitis,and S100βand electroencephalogram can be used as important indicators for screening severe hand-foot-mouth disease.
