摘  要: 目的探讨谷氨酰胺辅助治疗早产儿坏死性小肠结肠炎的疗效及对患儿肿瘤坏死因子-α水平的影响。方法选取我院2013年1月~2018年6月收治的早产儿坏死性小肠结肠炎患儿40例,随机数字表法分为对照组和研究组,对照组患儿给予抗感染、监护、防治并发症、胃肠外营养等常规治疗,研究组在对照组的基础上应用谷氨酰胺辅助治疗。对比两组患者谷氨酰胺治疗有效率、肿瘤坏死因子-α水平以及不良应发生率。结果研究组患者谷氨酰胺治疗有效率明显高于对照组,差异有统计学意义(P<0.05);研究组患者治疗后2d、4d、7d肿瘤坏死因子-α水平明显高于对照组,差异有统计学意义(P<0.05);研究组患者不良反应发生率显著低于对照组,差异有统计学意义(P<0.05)。结论早产儿坏死性小肠结肠炎属于消化道疾病,谷氨酰胺能有效辅助治疗早产儿坏死性小肠结肠炎,作为一种特殊的营养物质,谷氨酰胺对细胞因子具有重要的调节作用,能有效平衡患者的肿瘤坏死因子-α水平。 Objective To investigate the effect of glutamine adjuvant therapy on necrotizing enterocolitis in preterm infants and its influence on the level of tumor necrosis factor-α. Methods A total of 40 children with necrotizing enterocolitis of preterm infants admitted in our hospital from January 2013 to June 2018 were enrolled. The patients were divided into the control group and the study group by random number table. The control group received conventional treatments including anti-infection and monitoring, prevention and treatment of complications, parenteral nutrition, etc. The study group was given glutamine adjuvant therapy based on the treatment of the control group. The effective rate of glutamine treatment, tumor necrosis factor-α level and adverse reaction rate between the two groups were compared. Results The effective rate of glutamine treatment in the study group was significantly higher than that in the control group, the difference was statistically significant(P<0.05). The level of tumor necrosis factor-α at 2 d, 4 d, 7 d in the study group was also significantly higher than that in the control group, the difference was statistically significant(P<0.05). The incidence of adverse reactions in the study group was significantly lower than that in the control group, the difference was statistically significant(P<0.05). Conclusion Necrotic enterocolitis in premature infants is a digestive tract disease. Glutamine can effectively treat necrotizing enterocolitis in premature infants. As a special nutrient, glutamine has an important regulatory effect on cytokines, which can effectively balance the patients' tumor necrosis factor-α level.
