摘　　要： 目的针对细胞毒药物不合理用药医嘱干预情况进行整理分析,促进临床合理用药。方法查阅配置室2018年2月—2018年7月间配置的所有细胞毒药物配液医嘱,通过查阅药品说明书、药学工具书、相关知识文献等进行审核、汇总分析。结果总配置量2 345组,审核出不合理医嘱50组,不合理率2.13%。除占比较高的给药速度不当(34.0%)、药物浓度错误(30.0%)、溶剂选择不当(18.0%)外,还存在药物混合配伍不合理等。结论细胞毒药物临床用药仍存在一定的不合理率。细胞毒药物种类繁多,且近年来随着用药方案与治疗方案的不断更新,还出现了诸如多药混合配伍、化疗泵装量、给药顺序不合理等情况,药师应主动干预,促进临床合理用药。 Objective To analyze the irrational cytotoxic drug orders and to promote rational use of cytotoxic drugs in clinic.Methods The cytotoxic drug orders from February2018to July2018were collected and analyzed according to the drug legal instructions,the pharmaceutical instrument books and the related papers.Results50groups of irrational orders were discovered in2345groups,and the irrational rate was2.13%.In addition to the improper drug delivery speed rate(34.0%),wrong drug concentration(30.0%)and improper solvent selection(18.0%),there still existed irrational mixing of drugs.Conclusion There are still some unreasonable rates in the clinical use of cytotoxic drugs.There are many kinds of cytotoxic drugs,and in recent years,with the updating of medication and treatment schemes,there are also some situations,such as multi-drug mixing,chemotherapy pump loading,irrational drug delivery sequence,etc.Pharmacists should take the initiative to intervene in order to promote rational drug use in clinic.
