脂肪干细胞复合HA/β-TCP与同种异体骨修复兔脊柱骨缺损的比较

摘  要: [目的]比较脂肪干细胞(adipose-derived stem cells,ADSCs)复合羟基磷灰石/β-磷酸三钙(hydroxyapatite/tricalcium phosphate,HA/β-TCP)与复合同种异体骨修复兔脊柱骨缺损的效果。[方法]新西兰大白兔36只制备脊柱骨缺损模型,随机分为3组。12只动物于骨缺损处植入HA/β-TCP/ADSCs复合材料(HTS组),12只于骨缺损处植入同种异体骨(bone allograft)/ADSCs复合材料(BAS组),另外12只造模后不作任何处理,设为空白对照组(Blank control)(BC组)。术后第4、8、12周分别拍摄X线片,术后第12周实验动物处死后进行标本大体观察、组织病理学观察。[结果]术后4、8和12周时,HTS组X线片评分最高,BAS组次之,BC组最低,三组间差异均有统计学意义(P<0.05)。大体标本骨缺损修复HTS组最佳,BAS组次之,BC组最差。组织学观察未见明显排异反应,HTS组成骨修复活动最佳,BAS组次之,BC组最差。[结论]ADSCs与HA/β-TCP复合或与同种异体骨复合均有较好的生物相溶性,但前者的骨缺损修复能力优于后者。 [Objective] To compare the outcomes adipose-derived stem cells (ADSCs) combined with hydroxyapatite / tri- calcium phosphate (HA/β-TCP) versus bone allograft for treatment of spinal bone defects in rabbits. [Method] Thirty-six New Zealand white rabbits were used to make spinal bone defect models and randomly divided into 3 groups. Of them, 12 animals had the bone defect implanted with a composite of the HMβ-TCP/ADSCs (the HTS group), the other 12 animals got the defect filled with a compound of bone allograft/ADSCs (the BAS group) , whereas the remaining 12 animals were kept the defect un- treated as a blank control (the BC group). The X-ray checks were taken at 4, 8 and 12 weeks after operation, and then the spi- nal samples were harvested for gross and histological observation at 12 weeks postoperatively. [Results] In term of radiographic score of Lane and Sandhu, the HTS group got the highest grades, the BAS group was in the middle and the BC group had the low- est grades sequentially, associated with statistically significant differences among the three group at 4, 8 and 12 weeks after op- eration (P〈O.05) . In addition, gross observation also showed healing quality at the bone defect area in sequence of HTS〉BAS〉 BC. Furthermore, histological inspection revealed vigorous osteogenesis in the HTS group, moderate osteogenic phenomenon in the BAS group and scant new bone formation in the BC group without considerable rejective reaction in any group. [Conclu- sion] Both HA/β-TCP/ADSCs and bone allograft/ADSCs composites have good biological compatibility, however, the former issuperior to the latter in osteogenic capacity.
