超声生物显微镜在诊断和治疗原发性闭角型青光眼中的应用价值
The value of ultrasound biomicroscopy application in the diagnosis and treatment of primary angle-closure glaucoma

摘  要: 目的:探讨超声生物显微镜(UBM)在诊断和治疗原发性闭角型青光眼中的应用价值。方法:选取2017年6月至2018年6月广东省东莞市人民医院收治的原发性闭角型青光眼患者96例(试验组),同期选取62例在本院行眼部检查的健康体检者为对照组,均采用UBM检查。比较两组及试验组不同疾病类型患者(原发性急性闭角型青光眼、原发性慢性闭角型青光眼)角膜厚度、房角开放距离500、晶状体厚度、睫状突厚度、前房深度、小梁睫状体距离、睫状体晶状体距离等UBM定量检测指标结果,试验组不同疾病类型患者睫状体位置、虹膜形态、周边巩膜厚度等UBM半定量检测指标结果,并观察试验组治疗前、后UBM定量检测指标结果。结果:试验组与对照组角膜厚度比较,差异无统计学意义(P>0.05);试验组晶状体与睫状突厚度均大于对照组,房角开放距离500、前房深度、小梁睫状体距离、睫状体晶状体距离均小于对照组(均P<0.05);试验组原发性急性闭角型青光眼患者房角开放距离500、前房深度、小梁睫状体距离均小于原发性慢性闭角型青光眼患者(均P<0.05);试验组不同疾病类型患者睫状体位置、巩膜形态、周边巩膜厚度比较有明显差异(P<0.05)。试验组治疗后晶状体与睫状突厚度均较治疗前减小,房角开放距离500、前房深度、小梁睫状体距离、睫状体晶状体距离较治疗前增加(P<0.05)。结论:UBM在原发性闭角型青光眼疾病类型、病情监测方面具有较好的应用价值,可为临床诊治提供一定依据。 Objective:To investigate the value of ultrasound biomicroscopy(UBM)in the diagnosis and treatment of primary angle-closure glaucoma.Methods:From June 2017 to June 2018,96 patients with primary angle-closure glaucoma in our hospital were selected as the experimental group.62 healthy subjects who underwent eye examination were selected as the control group.The UBM quantitative test results such as corneal thickness,angle of opening angle 500,lens thickness,ciliary thickness,anterior chamber depth,trabecular ciliary body distance and ciliary body lens distance were compared between the two groups and patients with acute and chronic angle-closure glaucoma.The UBM semi-quantitative test results including ciliary body position,iris morphology and peripheral scleral thickness were compared between the patients with acute and chronic angle-closure glaucoma.The UBM quantitative test results in the experimental group were observed before and after treatment.Results:The results of UBM quantitative measurement showed that there was no significant difference in corneal thickness between the experimental group and the control group(P>0.05).The thickness of the lens and ciliary process in the experimental group was higher than that of the control group,and angle-opening distance 500,anterior chamber depth,trabecular ciliary body distance,and ciliary body lens distance were lower(P<0.05),and the decreases were more marked in the patients with primary acute angle-closure glaucoma.The ciliary body position,scleral morphology and peripheral scleral thickness were significantly different between the patients with different disease types in the experimental group(P<0.05).The thickness of the lens and ciliary process was decreased,while the angle-opening distance 500,the depth of the anterior chamber,the distance of the trabecular ciliary body,and the distance of the ciliary body lens were increased after treatment in the experimental group(P<0.05).Conclusion:UBM was valuable in the typing and monitoring of primary angle closure glauc
