实时超声在神经外科手术中的应用

摘  要: 目的探讨术中实时超声应用于神经外科手术的可行性、准确性和优缺点。方法分析实时超声下手术治疗颅内病变患者的术中超声图像表现及其特点,观察病灶检出率、病变切/清除率及术后3个月患者临床症状。结果实时超声监测病变检出率达100%。8例为非急诊手术,其中病变全切或近全切6例,大部分切除2例;另6例急诊手术的颅内血肿均完全清除。本组病变全切/清除率为85.7%,无死亡及颅内感染病例。结论实时超声监测应用于神经外科手术具有经济、安全、简便等优点,能缩短手术时间和减轻脑损伤。 Objective To explore the feasibility, accuracy and characteristics of the application of intraoperative ultrasound imaging in neurosurgical operation. Methods The intraoperative ultrasound images of patients with intracranial lesions treated by real-time ultrasound and their characteristics were analyzed. The detection rate of lesions and the rate of lesion resection/clearance as well as the clinical symptoms of patients 3 months after surgery were observed. Results The detection rate of lesions by intraoperative ultrasonography is 100%. There were 8 cases with non-emergent surgery, of which 6 cases had total or nearly total resection and 2 cases had majority resection. Another 6 cases underwent emergency operation had the intracranial hematoma completely cleared. The lesion resection/clearance rate of this group was 85.7%. There was no death case and the case of intracranial infection. Conclusion Application of intraoperative real-time ultrasonic monitoring is economical, safe, practical and simple. It can shorten operation time and reduce brain damage.
