1型糖尿病患者血清B细胞活化因子水平改变及其临床意义

摘  要: 目的:探讨1型糖尿病(T1DM)患者血清B细胞活化因子(BAFF)水平的变化及其临床意义。方法:采用酶联免疫吸附法(ELISA)检测26例T1DM患者、25例2型糖尿病(T2DM)患者、20例健康体检者血清BAFF水平。比较三组间差异,探讨血清BAFF与患者年龄、病程、胰岛功能、糖化血红蛋白、血脂等的关系。结果:血清BAFF水平T1DM组高于T2DM组及对照组(297. 54 vs. 218. 80 vs. 199. 20,F=4. 257,P <0. 05);T2DM组与对照组则差异无统计学意义(P> 0. 05); T1DM组血清BAFF水平与糖化血红蛋白(HbA1c)呈正相关(r=0. 613,P=0. 001),与空腹C肽(FCP)呈负相关(r=-0. 506,P=0. 008); T1DM组血清BAFF水平与总胆固醇(TC)、甘油三酯(TG)呈正相关(r=0. 413,0. 581,P=0. 036,0. 002);多元逐步法回归分析得出的回归方程为:FCP=0. 494-0. 001×BAFF水平;其中R2=0. 256,F=8. 241,P=0. 008。此方程表明BAFF水平与FCP呈弱负相关。结论:BAFF水平可能与T1DM患者血糖升高、C肽下降密切相关,并BAFF水平可能与T1DM患者血脂紊乱相关。 Objective: To investigate the serum level and clinical significance of B cell-activating factor belonging to the TNF family( BAFF) in patients with type 1 diabetes mellitus( T1 DM). Methods: 26 T1 DM patients,25 T2 DM patients and 20 healthy subjects were enrolled in our study. Serum BAFF were tested with emzyme-linked immunosorbent assay( ELISA). The differences between the three groups were compared to discuss the relationship between serum BAFF and age,course of disease,islet function,glycosylated hemoglobin,blood lipids and so on.Results: Serum BAFF level in T1 DM group were higher than that in T2 DM group and the control group( 297. 54 vs218. 80 vs 199. 20,F = 4. 257,P〈0. 05). Serum BAFF level in T2 DM group was higher than that of the control group( P〈0. 05); 2. In T1 DM group,the serum BAFF level were positively correlated with HbA1 c( r =-0. 506,P = 0. 008) while negatively correlated with FCP( r =-0. 506,P = 0. 008); serum BAFF level were positively correlated with TC and TG( r =-0. 413,0. 581,P = 0. 036,0. 002); 3. The regression equation of multivariate stepwise regression analysis was as follow: FCP = 0. 494-0. 001 × BAFF; R^2= 0. 256,F = 8. 241,P = 0. 008. This equation showed that the serum BAFF level were negatively correlated with FCP. Conclusion: Serum BAFF level may be closely related with elevated blood glucose and decreasing C-peptide in T1 DM patients,which may be associated with dyslipidemia in T1 DM patients.
