摘　　要： 目的观察切开引流加内口挂线术与内口直接切开术治疗肛周脓肿的临床效果。方法选择2013年2月至2016年12月在本院住院治疗的100例肛周脓肿患者作为研究对象,按照随机数字表法分成两组,每组各50例。观察组行切开引流加内口挂线术,对照组行肛周脓肿切开引流内口直接切开术。比较两组手术基本情况、术后疼痛、治疗效果及术后肛瘘发生情况。结果观察组手术时间长于对照组(P<0.05),观察组切口长度、住院时间及创面愈合时间均短于对照组(均P<0.05)。两组术后1 d、1 w及2 w时VAS评分均低于术前(均P<0.05),且观察组在各时点均低于对照组(均P<0.05)。观察组治愈率优于对照组(P<0.05),术后半年复发率和肛瘘发生率均低于对照组(均P<0.05)。结论切开引流加内口挂线术较内口直接切开术治疗肛周脓肿疗效更为显著,术后患者恢复更快,且有助于减少术后肛瘘形成。 Objective To investigate the clinical effect of incision and drainage plus seton stitch and direct incision to treat perianal abscess. Methods 100 patients with perianal abscess hospitalized and treated in our hospital from February 2013 to December 2016 were selected as study subjects. They were assigned into two groups using the random number table. The treatment group received the incision and drainage plus seton stitch and the control group direct incision. Indices related to operation, postoperative pain, therapeutic effect and postoperative anal fistula were compared between the two groups. Results The operation time was significantly longer in the treatment group than in the control group（P〈0.05）. Hospitalization duration, length of incision and time to wound healing were significantly shorter in treatment group than in the control group（P〈0.05）. In both groups, VAS pain scores 1 day, 1 week and 2 weeks after operation were significantly lower than that before operation（P〈0.05）. At each time point after operation, VAS pain scores were significantly lower in the treatment group than that in the control group（P〈0.05）. The cure rate was higher in the treatment group than in the control group（P〈0.05）. The recurrence rate and anal occurrence of fistula rate 6 months after operation were significantly lower in the treatment group than in the control group（P〈0.05）. Conclusion Incision and drainage plus seton stitch is better than direct incision in the treatment of perianal abscess. It is associated with faster recovery after operation and reduces the risk of postoperative fistula.
