摘　　要： 目的观察益气除痰散结法对Lewis肺癌小鼠的抑瘤作用及其机制。方法复制转移性Lewis小鼠肺癌模型,将模型小鼠随机分为益气除痰散结法中药复方高、中、低(21.45,85.80,171.60 g·kg^(-1))剂量组,环磷酰胺组(CTX,0.02 g·kg^(-1))和模型组,每组各10只,观察肿瘤增长体积、瘤体质量、免疫器官指数、肺转移率,并采用免疫组化方法结合计算机图像析技术,观察小鼠肿瘤组织中的血管内皮生长因子(VEGF)表达的差异。结果模型组小鼠平均瘤体质量为(2.44±0.52)g,提示肿瘤接种成功且生长良好;接种Lewis肺癌瘤株细胞后各组小鼠皮下肿瘤体积随时间增长而变化,其中中药复方高剂量组、环磷酰胺组的肿瘤体积增长显著低于模型组和中药复方低剂量组(P<0.01)。与模型组比较,中药复方高、中剂量组均有抑瘤作用(P<0.01,P<0.05),且明显增加胸腺及脾脏指数(P<0.01),对肺转移灶也有抑制作用(P<0.05,P<0.01)。从HE染色的病理切片中发现,在应用益气除痰散结法中药复方治疗后,小鼠肿瘤组织的病理学表现与模型组比较有明显的差异。免疫组化法检测,肿瘤微血管内皮细胞被染成棕黄色,VEGF主要表达肿瘤细胞核和胞浆内,肿瘤细胞亦有少量染色,阳性染色的肿瘤细胞大多位于液化坏死区,浸润的淋巴细胞有表达,血管内皮细胞呈弱阳性染色。但益气除痰散结法中药复方高、中、低剂量组VEGF值与模型组比较,差异无统计学意义(P>0.05)。结论益气除痰散结法能明显抑制荷瘤小鼠局部肿瘤的生长及肺转移,其抗肺癌作用的机理可能与促进肿瘤细胞凋亡、提高细胞分化程度,增强免疫力有关,但与VEGF靶点的关系不大。 Objective To observe the effect of Yiqi Chutan Sanjie therapy on advanced non-small cell lung cancer and discuss its partial mechanism.Methods The mice replicated with metastatic Lewis lung cancer model were randomly divided into traditional Chinese medicine high,middle and low dose groups(171.60,85.80,21.45 g·kg^-1),chemotherapy group(cyclophosphamide,CTX 0.02 g·kg^-1)and model group(10 rats for each group).The tumor growth,tumor volume,tumor weight,immune organ index,lung metastasis rate were measured;the difference of vascular endothecial growth factor(VEGF) expression in mouse tumor tissues was obtained by immunohistochemical method and computer-assisted image processing analysis.Results The tumor weight of rats in model group is(2.44 ±0.52 g),indicating the success of model set up.The tumor volume of rats in all groups increased with time,and the increase of the tumor volumes in rats of traditional Chinese medicine high dose group and in CTX group were significantly lower than those of rats in model group and in traditional Chinese medicine low dose group(P < 0.05).Compared with the model group,traditional Chinese medicine high and middle dosages exhibited tumor-inhibition effects(P < 0.01,P <0.05),which had significantly increased thymus indexes and spleen indexes(P < 0.01),and inhibitory effect on pulmonary metastasis focuses(P < 0.05,P < 0.01).The microscopic analysis showed that in tumor cells,the tumor microvascular endothelial cells were stained brown and the VEGF was mainly expressed in the nucleus and cytoplasm,with a small amount of staining in tumor cells;the positive stained tumor cells were mostly located in the liquefaction necrosis zone;the expression was also detected in infiltrated lymphocytes;and weak positive staining was shown in vascular endothelial cells.However,no significantly difference of VEGF expression was detected between traditional Chinese medicine groups and model group(P > 0.05).Conclusion Yiqi Chutan Sanjie therapy could significantly inhibit the growth of local tumor and
