摘　　要： 目的探索碳酸氢钠与血必净注射联合治疗急性重度敌敌畏中毒患者的临床效果。方法在2018年9月-2019年5月治疗急性重度敌敌畏中毒患者病例中方便选取72例,按就诊顺序随机分为两组,各36例。对照组采用常规治疗方法,观察组采用碳酸氢钠联合血必净注射治疗,观察对比两组患者治疗效果。结果观察组总有效率为94.44%,明显高于对照组的75.00%,差异有统计学意义(P<0.05);两组患者治疗1 d后HCO3-、APACHEⅡ评分、动脉血PH值、血清hsCRP等指标及胆碱酯酶活性相比,差异无统计学意义(P>0.05),治疗5 d后观察组APACHEⅡ评分、血清hs CR水平、胆碱酯酶活性和辅助呼吸时间、ICU住院时间及阿托品用量、并发症发生率均优于对照组,差异有统计学意义(P<0.05)。结论予以急性重度敌敌畏中毒患者碳酸氢钠与血必净注射联合治疗效果显著,可有效提高胆碱酯酶活性,缩短住院时间,促进康复,值得推广。 Objective To explore the clinical effect of sodium bicarbonate combined with Xuebijing injection in the treatment of patients with acute severe dichlorvos poisoning.Methods From January 2018 to January 2019,Convenient selection of 72 patients with acute severe dichlorvos poisoning were selected and randomly divided into control group and observation group according to the order of treatment,36 cases each.The control group was treated with conventional treatment.The observation group was treated with sodium bicarbonate combined with Xuebijing injection.The therapeutic effects of the two groups were observed and compared.Results The observation group total effective rate was 94.44%,higher than the observation group 75.00%,the difference was statistically significant(P<0.05).There was no significant difference in HCO3-,APACHE II score,arterial blood pH,serum hs CRP and cholinesterase activity between the two groups after treatment for 1 d(P>0.05).After 3 days of treatment,the APACHE II score,serum hsCR level,cholinesterase activity and assisted breathing time,ICU hospitalization time,atropine dosage and complications were observed.The incidence was better than the control group,the difference was statistically significant(P<0.05).Conclusion The combination of sodium bicarbonate and Xuebijing injection in patients with acute severe dichlorvos poisoning has significant effects,which can effectively improve cholinesterase activity,shorten hospitalization time,promote rehabilitation,and is worth promoting.
