摘　　要： 目的:探讨阿尔茨海默病患者Th17/Treg平衡的改变情况及其作用机制,为临床提供一定的理论参考。方法:选取2018年2月-2019年10月笔者所在医院收治的58例65岁以上阿尔茨海默病患者设为试验组,同时选取同期防保门诊与体检中心收治的30例65岁以上健康者,设为对照组。采用ELISA法检测两组外周血TGF-β、IL-6、IL-23、IL-10和IL-17水平,采用流式细胞术检测两组外周血Th17和Treg细胞比率。结果:试验组外周血TGF-β和IL-10水平低于对照组,IL-6、IL-23和IL-17水平高于对照组,差异均有统计学意义(P<0.05)。试验组外周血Th17细胞比率高于对照组,Treg细胞比率低于对照组,Th17/Treg高于对照组,差异有统计学意义(P<0.05)。Spearson相关性分析发现,Th17细胞比率和Treg细胞比率呈负相关,Th17/Treg与IL-6、IL-23、IL-17水平呈正相关,Th17/Treg与TGF-β和IL-10水平负相关(P<0.05)。结论:阿尔茨海默病患者存在Th17/Treg平衡改变的现象。 Objective:To explore the change of Th17/Treg balance in Alzheimer disease patients and its mechanism action,so as to provide certain theoretical reference for clinical practice.Method:A total of 58 patients over 65 years old with Alzheimer disease admitted in our hospital from February 2018 to October 2019 were selected as the experimental group,and 30 healthy patients over 65 years old who were admitted in the prevention clinic and physical examination center at the same time were selected as the control group.The levels of TGF-β,IL-6,IL-23,IL-10 and IL-17 in the peripheral blood of the two groups were detected by ELISA,and the ratio of Th17 and Treg cells in the peripheral blood of the two groups was measured by flow cytometry.Result:The levels of TGF-βand IL-10 in the peripheral blood of the experimental group were lower than those in the control group,and the levels of IL-6,IL-23 and IL-17 were higher than those in the control group,and the differences were statistically significant(P<0.05).The ratio of Th17 cells in the peripheral blood of the experimental group was higher than that of the control group,the ratio of Treg cells was lower than of the control group,and the value of Th17/Treg was higher than the control group,and the difference was statistically significant(P<0.05).Spearson correlation analysis found that the ratio of Th17 cells was negatively correlated with Treg cells.Th17/Treg was positively correlated with IL-6,IL-23,and IL-17 levels,and Th17/Treg was negatively correlated with TGF-βand IL-10 levels(P<0.05).Conclusion:The balance of Th17/Treg is changed in patients with Alzheimer disease.
