剖宫产术后再次妊娠孕妇采用阴道分娩方式的可行性及对母婴结局的影响

摘  要: 目的探讨剖宫产术后再次妊娠孕妇采用阴道分娩方式的可行性及对母婴结局的影响。方法选取2016年1月~2018年9月在我院分娩的无剖宫产史50例孕妇作为对照组,择同期在我院分娩的剖宫产术后再次妊娠50例孕妇作为研究组,比较两组产程时间、出血量、住院时间、新生儿体质量、新生儿1min、5min时Apgar评分、新生儿神经行为功能评分、产妇产后并发症发生率、新生儿并发症发生率。结果两组产程时间、出血量、住院时间、新生儿体质量、新生儿1min、5min时Apgar评分、新生儿神经行为功能评分、产妇产后并发症发生率、新生儿并发症发生率比较,差异无统计学意义(P>0.05)。结论剖宫产术后再次妊娠孕妇采用阴道分娩方式是切实可行的,对母婴结局无明显影响,可降低再次剖宫产率。 Objective To explore the feasibility of vaginal delivery in pregnant women with re-pregnancy after cesarean section and its effect on maternal and infant outcomes. Methods 50 pregnant women with no cesarean section who delivered in our hospital from January 2016 to September 2018 were selected as the control group.50 pregnant women who had re-pregnancy after cesarean delivery in our hospital during the same period were selected as the study group.The duration of labor, blood loss,hospital stay,the body weight of newborns,the Apgar scores at 1 min and 5 min,the score of neonatal neurobehavioral function,the incidence of maternal postpartum complications and the incidence of neonatal complications were compared between two groups. Results There was no statistically significant difference in duration of labor, blood loss,hospital stay,the body weight of newborns,the Apgar scores at 1 min and 5 min,the score of neonatal neurobehavioral function,the incidence of maternal postpartum complications and the incidence of neonatal complications between two groups (P > 0.05). Conclusion It is feasible to adopt vaginal delivery in pregnant women who have re-pregnancy after cesarean section.It has no significant effect on maternal and infant outcomes and can reduce the rate of re-cesarean section.
