盐酸氨溴索联合异丙托溴铵雾化吸入治疗老年呼吸机相关性肺炎临床观察

摘  要: 目的观察盐酸氨溴索联合异丙托溴铵雾化吸入治疗老年呼吸机相关性肺炎的临床效果。方法 68例机械通气治疗过程中并发呼吸机相关性肺炎的老年患者,按随机数表法分为对照组和观察组,各34例。两组患者均行抗感染及对症治疗,对照组在上述基础上采用异丙托溴铵雾化治疗,观察组在对照组基础上加用盐酸氨溴索治疗,比较两组相关临床指标及疗效。结果观察组通气时间为(8.3±1.6)d,支气管镜吸痰次数为(3.6±0.4)次,白细胞计数为(9.6±2.4)×109/L,明显优于对照组的(13.8±3.2)d、(6.8±1.3)次、(14.3±2.8)×109/L,差异有统计学意义(P<0.05)。观察组治疗总有效率为94.1%,明显高于对照组的73.5%,差异有统计学意义(P<0.05)。结论给予老年呼吸机相关性肺炎患者盐酸氨溴索联合异丙托溴铵雾化治疗可提高治疗效果,缩短通气时间,促进患者恢复。 Objective To observe the clinical effect of ambroxol hydrochloride combined with ipratropium bromide atomization inhalation in the treatment of senile ventilator associated pneumonia. Methods A total of 68 senile patient complicated with ventilator associated pneumonia in the process of mechanical ventilation treatment were divided by random number table method into control group and observation group, with 34 cases in each group. Both group received anti-infection and symptomatic treatment, and the control group also received ipratropium bromide atomization inhalation treatment. The observation group received ambroxol hydrochloride for treatment on the basis of the control group. Comparison were made on related clinical indicators and curative effect in two groups. Results The observation group had duration of ventilation as (8.3±1.6) d,bronchoscopy sputum suctions as (3.6±0.4) times, white blood cell count as (9.6±2.4)×109/L, which were all better than (13.8±3.2) d, (6.8±1.3) times and (14.3±2.8)×109/L in the control group, and their difference had statistical significance (P<0.05). The observation group had obviously higher total treatment effective rate as 94.1% than 73.5% in the control group, and the difference had statistical significance (P<0.05). Conclusion Combination of ambroxol hydrochloride and ipratropium bromide atomization inhalation can improve treatment effect, shorten duration of ventilation and promote recovery of patients.
