摘 要: 目的:探讨非高密度脂蛋白胆固醇(non-HDL-C)水平与女性人群动脉僵硬度的关系。方法:以肱踝脉搏波传导速度(baPWV)≥1 700 cm/s为标准,1 286例参加健康体检的女性分为动脉硬化组(n=133)和对照组(n=1 153)。应用Pearson相关及多因素回归分析各血脂指标对baPWV的影响,受试者工作特征(ROC)曲线讨论non-HDL-C对动脉硬化的诊断价值。结果:(1)动脉硬化组的年龄、体质指数(BMI)、收缩压、舒张压、空腹血糖、尿酸、肌酐、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、non-HDL-C水平及高血压、糖尿病人数比例均高于对照组(均P<0.01)。(2)Pearson相关分析显示baPWV与TC、TG、LDL-C及non-HDL-C呈正相关(r分别为0. 331,0.320,0.280,0. 357,均P <0. 01)。(3)多元回归分析表明,non-HDL-C水平与baPWV独立相关。(4)non-HDL-C的ROC曲线下面积最大(0.746,95%CI:0.705~0.788,P<0.001)。结论:non-HDL-C是动脉僵硬度增加的独立预测因子。与常规血脂指标相比,non-HDL-C与女性动脉硬化的相关性更为密切。 Objective:To explore the relationship between non-high density lipoprotein cholesterol(non-HDL-C) and arterial stifness in women.Methods:Based on brachial-ankle pulse wave velocity(ba PWV) ≥1 700 cm/s,1 286 female subjects were divided into arterial stifness group(n=133) and control group(n=1 153).Pearson correlation and multivariate regression analysis were used to evaluate the relationship between lipid parameters and baPWV.The predictive value of non-HDL-C for arterial stifness was analyzed by receiver operating characteristic(ROC) curve.Results:①Age,BMI,blood pressure,fasting plasma glucose,uric acid,creatinine,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),non-HDL-C,history of diabetes and hypertension in arterial stiffness group were higher than those of control group(all P<0.01).②Pearson correlation showed that TC,TG,LDL-C and non-HDL-C were positively correlated with baPWV(r=0.331,0.320,0.280,0.357,all P<0.01).③Multivariate regression analysis demonstrated that non-HDL-C was independently correlated with baPWV.④The area under the ROC of non-HDL-C was the largest among lipid parameters(0.746,95%CI:0.705~0.788,P<0.001).Conclusion:Non-HDL-C was independently correlated with baPWV and it was superior to routine lipid parameters for estimating arterial stiffness in women.
