放射性脑损伤发病机制与治疗方法研究进展

摘  要: 放射性脑损伤是头颈部肿瘤放射治疗后并发症之一,严重影响患者生存质量。目前关于放射性脑损伤的发病机制尚未完全清晰,可能与血管损伤、胶质细胞损伤及免疫炎性反应有关。放射性脑损伤的传统治疗方法如高压氧、糖皮质激素、手术等可有效缓解患者临床症状,但无法逆转脑损伤进程。随着对放射性脑损伤的研究不断深入,贝伐单抗、神经生长因子、神经节苷脂及干细胞移植等新方法成功应用于临床。本文就放射性脑损伤发病机制和治疗方法的研究进展作一综述。 Radiation-induced brain injury (RBI)is one of the complications of radiotherapy of head and neck tumors,and seriously lowers the quality of life of patients.At present,the pathogenesis of RBI has not yet been fully elucidated,and it might be correlated with vascular injury,glial cell injury and immune inflammatory response.The traditional treatment of RBI,as hyperbaric oxygen,glucocorticoid and surgery,can not reverse the process of brain injury although it can effectively alleviate the clinical symptoms of RBI.With the further research on RBI,new methods such as bevacizumab, nerve growth factor,ganglioside and stem cell transplantation have been successfully applied in treatment.This paper reviews the research progress of the pathogenesis and treatment of RBI.
