摘　　要： 目的探讨老年脑梗死患者心肌酶水平与脑梗死严重程度临床意义情况。方法选取清远市人民医院脑科三区2016年2月~2018年2月收治的老年脑梗死患者157例作为观察组,选取同期健康体检者100例作为对照组,根据脑梗死严重程度,分为轻度组77例、中度组60例、重度组20例,根据老年脑梗死患者是否具有意识障碍进行分组,意识障碍组60例,无意识障碍组97例。结果观察组老年脑梗死患者谷草转氨酶(AST)、血清肌酸激酶(CK)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)均高于对照组,中度、重度老年脑梗死患者AST、CK、LDH、CK-MB均高于轻度患者,重度患者AST、CK、LDH、CK-MB均高于中度患者,意识障碍组患者AST、CK、LDH、CK-MB均高于无意识障碍组,差异均有统计学意义(P<0.05)。结论急性脑梗死可诱发心肌酶谱各项指标升高,心肌酶谱中各项指标(AST、CK、LDH、CK-MB)可以用来评估老年脑梗死患者病情严重程度,指标越高,脑梗死患者病情可能越严重,为指导临床治疗提供了理论依据。 Objective To investigate the clinical significance of myocardial enzyme level and severity of cerebral infarction in elderly patients with cerebral infarction.Methods 157 elderly patients with cerebral infarction admitted and treated in District 3,Department of Brain,Qingyuan People’s Hospital from February 2016 to February 2018 were selected as the observation group,while 100 healthy examined patients in the same period were selected as the control group.According to the severity of cerebral infarction,there were 77 patients in the mild group,60 patients in the moderate group,and 20 patients in the severe group.According to whether the elderly patients with cerebral infarction have consciousness disorder,they were divided into the consciousness disorder group(60 patients)and the unconsciousness disorder group(97 patients).Results The aspartate aminotransferase(AST),serum creatine kinase(CK),lactic dehydrogenase(LDH),and creatine kinase isoenzyme(CK-MB)in the elderly patients with cerebral infarction of the observation group were all higher than those of the control group;AST,CK,LDH and CK-MB in the elderly patients with moderate and severe cerebral infarction were all higher than those in the mild patients,AST,CK,LDH and CK-MB in the severe patients were all higher than those in the moderate patients,and AST,CK,LDH and CK-MB in the patients of the consciousness disorder group were all higher than those of the unconsciousness disorder group,with statistically significant difference(P<0.05).Conclusion Acute cerebral infarction can induce the elevation of the indexes in the myocardial enzymogram.The indexes(AST,CK,LDH and CK-MB)in the myocardial enzymogram can be used to evaluate the severity of condition in elderly patients with cerebral infarction.The higher the index,the more severe the condition of the patients with cerebral infarction may be,providing a theoretical basis for guiding clinical treatment.
