摘　　要： 目的观察中医辨证分型联合振动疗法治疗小儿脑性瘫痪的临床疗效。方法选取2016年11月—2018年1月我院66例脑性瘫痪患儿,随机分为两组,各33例。对照组采用常规药物、康复训练及护理措施,观察组采用振动疗法联合中医辨证分型治疗,两组均持续治疗2个月。治疗前后两组患儿行经颅彩色多普勒(TCD)检查脑血流变化,比较两组患儿临床疗效、粗大运动功能(GMFM)量表和精细运动功能(FMFM)量表评分以及记录不良反应发生情况。结果对照组总有效率为75.76%,低于观察组的93.94%,差异有统计学意义(P <0.05)。治疗前两组患儿GMFM量表评分、FMFM量表评分及大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)血流速度比较,差异无统计学意义(P>0.05)。治疗后对照组和观察组GMFM量表评分、FMFM量表评分、ACA、MCA、PCA较治疗前升高,且观察组高于对照组,差异均有统计学意义(P <0.05)。结论中医分型辨证治疗联合振动疗法可以提高小儿脑性瘫痪的治疗效果,改善运动功能和脑微循环。 Objective To explore the effect of traditional Chinese medicine(TCM)syndrome differentiation and vibration therapy for cerebral palsy in children.Methods Sixty-six children with cerebral palsy in our hospital from November 2016 to January 2018 were selected,and randomly divided into two groups:control group(n=33)treated with routine treatment,rehabilitation training and nursing care,and treatment group(n=33)treated with TCM syndrome differentiation and vibration therapy for 2 months.The changes of cerebral blood flow were examined by transcranial color Doppler(TCD)before and after treatment.The clinical efficacy,the gross motor function(GMFM),and the fine motor function(FMFM)were compared between two groups,and the adverse reaction was recorded.Results The total clinical effective rate was significantly higher in treatment group than that in control group(93.94%vs 75.76%,P<0.05).There was no significant difference in the scores of GMFM scale and FMFM scale,and levels of anterior cerebral artery(ACA),middle cerebral artery(MCA),and posterior cerebral artery(PCA)between two groups before treatment(P>0.05).After treatment,the scores of GMFM scale and FMFM scale,and levels of ACA,MCA,and PCA were higher than those before the treatment,which were higher in treatment group than that in control group.The difference was statistically significant(P<0.05).Conclusion TCM syndrome differentiation and vibration therapy can improve the therapeutic effect of cerebral palsy in children,improve motor function and brain microcirculation.
