摘　　要： 目的:探讨结直肠癌磁共振表观弥散系数表现与核苷酸切除修复交叉互补基因(ERCC)相关性。方法:选取2016年10月至2018年6月收治的120例结直肠癌患者进行研究,根据病理结果分为高分化组、中分化组、低分化组,每组各40例,并选取结直肠癌癌旁黏膜组织的正常肠壁组织40例作为对照组,采用磁共振成像(MRI)检查弥散系数表现,并与手术后常规病理免疫组化ERCC检测结果进行相关性分析。结果:高分化、中分化、低分化组ADC值和ERCC表达水平之间存在明显差异(P<0.05);随着结直肠癌肿瘤组织ERCC表达增加,其ADC值降低,结直肠癌ADC值与ERCC表达程度呈负相关(r=-0.598,P=0.000);高分化(r=-0.688,P=0.000)、中分化(r=-0.698,P=0.000)、低分化(r=-0.741,P=0.000)与ERCC的表达水平均呈为负相关。结论:结直肠癌的ADC值能从分子水平反映了肿瘤细胞的增殖程度,ADC值低的患者,其ERCC处于高表达水平,结直肠癌ADC值的变化可以作为结直肠癌术前的恶性程度进行诊断及临床疗效的判断依据。 Objective:To investigate the correlation between the apparent diffusion coefficient(ADC)of magnetic resonance imaging(MRI)and the nucleotide excision repair cross-complementing gene(ERCC)in colorectal cancer.Methods:One hundred and twenty patients with colorectal cancer admitted between October 2016 and June 2018 were enrolled in the study,and according to pathological findings,divided into high-,intermediate-,and low-differentiation groups(n=40 each).Forty samples of normal intestinal wall tissue adjacent to colorectal cancer were selected as the control group.MRI was used to measure the performance of ADC which was then included in a correlation analysis along with the results of ERCC test as shown by postoperative immunohistochemistry.Results:There were significant differences in ADC values and ERCC expression levels among the high-,intermediate-,and low-differentiation groups(P<0.05).Along with higher expression of ERCC in colorectal cancer tissues,the ADC value decreased.The ADC value of colorectal cancer was negatively correlated with ERCC expression(r=-0.598,P=0.000).As for the degree of differentiation of colorectal cancer,high-differentiation(r=-0.688,P=0.000),intermediate-differentiation(r=-0.698,P=0.000)and low-differentiation(r=-0.741,P=0.000)were negatively correlated with ERCC expression.Conclusion:ADC value of colorectal cancer may reflect the proliferation of tumor cells at the molecular level.The patients with low ADC value may have high expression level of ERCC.The change in ADC value of colorectal cancer is useful for evaluating the clinical malignancy and efficacy in colorectal cancer.
