摘　　要： 目的分析微创钬激光碎石术在输尿管结石治疗中应用价值及炎症因子的影响。方法选取2017年1月~2018年12月我院收治的108例输尿管结石患者为研究对象,按随机数字表法分为观察组和对照组,各54例。对照组以传统开放性手术进行取石,观察组则采用微创钬激光碎石术取石,比较两组手术指标、炎症反应变化和术后并发症发生情况。结果观察组手术时间、住院时间、离床活动时间和术后肠胃功能恢复时间短于对照组,差异有统计学意义(P<0.05);观察组术中出血量少于对照组,差异有统计学意义(P<0.05);观察组术后IL-6、IL-10和TNF-α水平低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论在输尿管结石治疗中实施微创钬激光碎石术可缩短患者手术时间和住院时间,降低术中出血量和并发症发生率,减少对炎症因子的影响,安全性高,值得应用。 Objective To analyze the value of minimally invasive holmium laser lithotripsy in the treatment of ureteral calculi and its influence on inflammatory factors.Methods 108 patients with ureteral calculi admitted to our hospital from January 2017 to December 2018 were enrolled in this study.The patients were divided into observation group and control group according to the random number table method,with 54 cases in each group.The control group was treated with traditional open surgery for stone extraction,while the observation group was treated with minimally invasive holmium laser lithotripsy for stone extraction.The surgical indexes,changes in inflammatory response and postoperative complications were compared between the two groups.Results The operation time,hospitalization time,time to out-of-bed activity and time to postoperative gastrointestinal function recovery in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The intraoperative blood loss in the observation group was less than that in the control group,and the difference was statistically significant(P<0.05).The levels of IL-6,IL-10 and TNF-αin the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The implementation of minimally invasive holmium laser lithotripsy in the treatment of ureteral calculi can shorten operation time and hospitalization time,decrease intraoperative blood loss and incidence of complications,reduce the impact on inflammatory factors,and has high safety,which is worthy of application.
