摘　　要： 目的分析研究降钙素原(PCT)和IL-6、IL-8、IL-10联合检测在高血压脑出血患者开颅手术后颅内感染诊断中的临床价值。方法选取2018年3月~2019年3月我院神经外科收治的78例高血压脑出血开颅手术后患者,根据患者是否出现术后颅内感染将其分为感染组(38例)和非感染组(40例),患者在开颅手术前及术后第1、3、5、7、10、14天均进行外周静脉血的PCT、白介素-6(IL-6)、IL-8、IL-10水平检测,分析比较两组患者外周血PCT和IL-6、IL-8、IL-10水平的差异。结果两组患者的一般资料、术前及术后第1天的血清PCT和IL-6、IL-8、IL-10水平比较,差异无统计学意义(P> 0.05)。感染组患者术后(3~14d)的血清中的PCT和IL-6、IL-8、IL-10水平均明显高于非感染组,差异有统计学意义(均P <0.05),且随着发病时间的延长PCT和IL-6、IL-8、IL-10水平逐渐增高。结论联合监测患者外周血的PCT和IL-6、IL-8、IL-10水平,对高血压脑出血患者开颅手术后颅内感染的早期诊断具有重要的意义,可作为颅内感染的早期诊断指标。 Objective To analyze clinical values of combined detection of procalcitonin(PCT) and interleukin-6, 8, 10 in diagnosis of intracranial infection after craniotomy in patients with hypertensive intracerebral hemorrhage. Methods 78 patients with hypertensive intracerebral hemorrhage who were admitted and treated in department of neurosurgery in our hospital from March 2018 to March 2019 were selected. According to whether the patients had postoperative intracranial infection, they were divided into infected group(38 cases) and non-infected group(40 cases).Patients were examined for PCT, interleukin-6(IL-6), IL-8, and IL-10 levels in peripheral venous blood before and 1, 3, 5, 7, 10, and 14 days after craniotomy. The differences of PCT and IL-6, IL-8 and IL-10 levels in peripheral blood of the two groups were analyzed and compared. Results There were no statistically significant differences in the general data, serum PCT and IL-6, IL-8, IL-10 levels before surgery and 1 day after surgery of the two groups(P > 0.05).The levels of PCT, IL-6, IL-8 and IL-10 in the serum after surgery(3-14 days) of the infected group were significantly higher than those in the non-infected group. The difference was statistically significant(P < 0.05).And with the prolongation of onset time, PCT and IL-6, IL-8, IL-10 levels gradually increased. Conclusion Combined detection of PCT and IL-6, IL-8, IL-10 levels in peripheral blood of patients is of great significance for early diagnosis of intracranial infection after craniotomy in patients with hypertensive intracerebral hemorrhage, which can be used as an early diagnostic index of intracranial infection.
