摘　　要： 目的分析住院患者的住院费用及其主要影响因素,为政府控制医疗费用的不合理增长提供依据。方法收集2014年深圳市属三级甲等综合性医院住院患者180 197例,采用描述性分析、秩和检验、多重线性回归分析方法对住院患者基本特征、费用构成及其影响因素进行统计分析。结果深圳市属三级甲等综合性医院的首要疾病种类是影响健康状态和保健机构接触的因素,慢性非传染性疾病仍是常见、重点疾病;住院费用构成不合理,药费、耗材费比例过高;住院天数、是否手术、年龄、是否输血、是否抢救、性别、是否有出院31 d内再入院计划、是否转科、医疗付费方式、婚姻状况、病例分型、入院途径是住院费用的影响因素。结论影响住院费用的因素是多方面的,需要政府、医院、患者共同参与,采取综合措施控制住院费用的不合理增长,以实现各方利益的均衡发展。 Objective To analyze the hospitalization expense of patients and key influencing factors,in order to provide the basis for the government to control the unreasonable growth of medical costs. Methods Information of 180,197 cases of hospitalized patients from tertiary municipal hospitals of Shenzhen was collected,descriptive analysis,rank sum test,multiple linear regression was used to analyze the basic characteristics of hospitalized patients,hospitalization expense structure and influencing factors. Results The primary types of diseases were the factors of influencing health status and contact with health-care institutions,but the common and priority disease was alsochronic non-communicable diseases in tertiary general hospitals of Shenzhen; The structure of hospitalization expense was unreasonable and the largest cost share was drugs and medical consumables,the hospitalization expense was related to the following factors： days of hospitalization,operation situations,age,blood transfusion situations,rescue,gender,discharged 31 days readmission plans,switch departments,medical payment methods, marriage, case types, admission ways. Conclusions Factors of influencing hospitalization expense is manifold,therefore,government,hospitals and patients should participate and take comprehensive measures to control the unreasonable growth of medical costs in order to achieve balanced development interests of all parties.
