乙型肝炎病毒感染患者HBV-DNA载量与血清及肝纤维化标志物的关系

摘  要: 目的探讨乙型肝炎病毒(HBV)感染患者HBV-DNA载量与血清及肝纤维化标志物的相关性。方法选取2012年1月-2015年12月医院收治的86例HBV感染患者,按照HBV血清标志物分为A组(大三阳)21例、B组(小三阳)35例、C组(除外大小三阳的HBsAg+表型)24例、D组(抗体阳性及全阴)6例,根据HBVDNA载量分为5个级别分为<10~3 IU/ml组、10~3~10~5 IU/ml组、10~5~10~6 IU/ml组、10~6~10~8 IU/ml组和>10~8IU/ml组。结果 A组HBV-DNA载量显著高于B组和C组,差异有统计学意义(P<0.05);C组HBV-DNA载量显著高于B组(P<0.05);透明质酸(HA)随着HBV-DNA复制水平的增高而增高,5个不同级别HBV-DNA载量组两两比较差异有统计学意义(P<0.05);层粘连蛋白(LN)、Ⅲ型前胶原肽(PⅢP)、Ⅳ型胶原(CⅣ)在<10~3IU/ml组、10~3~10~5 IU/ml组、10~5~10~6 IU/ml组中随着患者血清HBV-DNA载量上升也随之递增,而在10~6~10~8 IU/ml组和>10~8 IU/ml组中随着血清HBV-DNA载量上升出现相对降低的现象。结论 HBV-DNA载量与HBV血清标志物不同模式组存在相关性,并与LN、PⅢP、CⅣ存在相关性,但不呈直线相关关系,与HA呈正相关。 OBJECTIVE To investigate the correlation between HBV-DNA load and serum and liver fibrosis markers in patients with hepatitis B virus infection.METHODS A total of 86 patients with hepatitis B virus infection in our hospital from Jan.2012 to Dec.2015 were selected.According to HBV serum markers,patients were divided into group A(big Sanyang,21 cases),group B(small Sanyang,35 cases),group C(other HBsAg+ phenotype,24 cases),and group D(antibody positive and overcast,6 cases).According to the HBV-DNA load,patients were divided into 5 levels:10-3 IU/ml group,10-3-10^5 IU/ml group,10^5-10^6 IU/ml group,10^6-10-8 IU/ml group,and 10-8 IU/ml group.RESULTS The HBV-DNA load in group A was significantly higher than that in group B and group C(all P〈0.05).The HBV-DNA load in group C was significantly higher than that in group B(P〈0.05).HA was increased with the increase of the HBV-DNA replication level,and there were significant differences of 5 different levels of HBV-DNA load groups compared with each other(all P〈0.05).LN,PⅢP and CⅣwere increased with the increase of serum HBV-DNA load in( 10-3 IU/ml)group,(10-3-10^5 IU/m)l group and(10^5-10^6 IU/ml)group of patients,while were decreased in(10^6-10-8 IU/ml)group and 10-8 IU/ml group with the increase of serum HBV-DNA load.CONCLUSION There is a correlation between HBV-DNA load and HBV serum markers,and a correlation between HBV-DNA load and LN,PⅢP and CⅣ,but not linear correla tion,and HBV-DNA load is positively correlated with HA.
