摘 要: 目的探讨T2DM患者血清载脂蛋白B(ApoB)、脂蛋白a(Lp-a)、TG和转铁蛋白(TRF)的变化与DR的相关性。方法回顾性分析150例T2DM住院患者临床资料,根据眼底检查结果分为单纯T2DM组80例和DR组70例,相关血清指标行独立样本t检验,对有统计学差异的指标行多元Logistic回归分析。结果与T2DM组比较,DR组糖尿病病程长,血清ApoB、TG、Lp-a和胱抑素(Cys)水平升高(P<0.05或P<0.01),血清TRF水平降低(P<0.01);Logistic回归分析结果显示,糖尿病病程、ApoB、TG及Lp-a为DR的危险因素(OR1.267、11.237、1.618、1.002,P<0.05或P<0.01),TRF为DR的保护性因素(OR0.432,P<0.01)。结论血清ApoB、TG、Lp-a和TRF代谢异常与DR关系密切,可能参与了DR的发生发展。 Objective To investigate the Correlation of serum apolipoprotein B (ApoB),triglyceride (TG),lipoprotein a(Lp-a) and transferrin(TRF) with diabetic retinopathy(DR) in type 2 diabetes mellitus (T2DM). Methods The clinical data of 150 patients with T2DM were retrospectively analyzed. 150 T2DM patients were divided into T2DM group (n=80) and DR group (n=70), according to their fundus photography. The relevant serum parameters were tested by t-test, and the statistically significant indexes were analyzed by logistic regression analysis. Results Compared with T2DM group, the course of diabetes was longer, the concentrations of ApoB,TG, Lpa and cystatin (Cys) were increased (P〈0.05 or P%0.01) ,serum TRF were decreased (P^0.01) in DR group. Logistic regression analysis showed that the course of diabetes, serum ApoB, TG and Lp-a were the risk factors of DR (OR 1. 267,11. 237,1. 618, 1. 002,P〈0.05 or P〈0.01),TRF was the protective factor of DR (OR 0.432, P〈0.01). Conclusion Abnormality of serum ApoB,TG, Lp-a and TRF is correlated with DR and might be involved in the development of DR.
