食管癌根治术后肠内免疫营养对抗肿瘤免疫应答、肠黏膜屏障功能的影响

摘  要: 目的:研究食管癌根治术后肠内免疫营养对抗肿瘤免疫应答、肠黏膜屏障功能的影响。方法:选择在我院接受食管癌根治术的102例患者并随机分为观察组和对照组,分别接受术后肠内免疫营养和常规肠内营养。手术前1d、手术后1d及7d时,检测外周血中免疫细胞标志物的表达量及血清中肠黏膜屏障功能损伤标志物的含量。结果:术后1d时,两组外周血中T-bet、NKG2D、NKp30、NKp44、NKp46的荧光强度均显著低于术前1d,外周血中GATA-3、Foxp3的荧光强度以及血清中DAO、Occludin、ZO-1、claudin-1的含量均显著高于术前1d;观察组术后7d时外周血中T-bet、NKG2D、NKp30、NKp44、NKp46的荧光强度均显著高于术后1d,外周血中GATA-3、Foxp3的荧光强度以及血清中DAO、Occludin、ZO-1、claudin-1的含量均显著低于术后1d;对照组术后7d时外周血中T-bet、GATA-3、Foxp3、NKG2D、NKp30、NKp44、NKp46的荧光强度与术后1d比较无显著性差异,血清中DAO、Occludin、ZO-1、claudin-1的含量均显著低于术后1d。结论:食管癌根治术后肠内免疫营养能够增强抗肿瘤免疫应答、改善肠黏膜屏障功能。 Objective:To study the effect of enteral immunonutrition after radical surgery for esophageal carcinoma on anti-tumor immune response and intestinal mucosal barrier function.Methods:A total of 102 patients who underwent radical surgery for esophageal carcinoma in our hospital were selected and randomly divided into observation group and control group who received postoperative enteral immunonutrition and routine enteral nutrition,respectively.One day before operation as well as 1dand 7dafter operation,peripheral blood immune cell marker expression and serum intestinal mucosal barrier injury marker levels were detected and compared.Results:One day after operation,peripheral blood T-bet,NKG2 D,NKp30,NKp44 and NKp46fluorescence intensity of both groups of patients were significantly lower than those 1dbefore operation while peripheral blood GATA-3and Foxp3 fluorescence intensity as well as serum DAO,Occludin,ZO-1and claudin-1levels were significantly higher than those 1dbefore operation;peripheral blood T-bet,NKG2 D,NKp30,NKp44 and NKp46fluorescence intensity of observation group 7dafter operation were significantly higher than those 1dafter operation while peripheral blood GATA-3and Foxp3 fluorescence intensity as well as serum DAO,Occludin,ZO-1and claudin-1levels were significantly lower than 1dafter operation;peripheral blood T-bet,GATA-3,Foxp3,NKG2 D,NKp30,NKp44 and NKp46fluorescence intensity of control group 7dafter operation were not significant different from those 1dafter operation,and serum DAO,Occludin,ZO-1and claudin-1levels were significantly lower than those 1dafter operation.Conclusion:Enteral immunonutrition after radical surgery for esophageal carcinoma can enhance the anti-tumor immune response and improve the intestinal mucosal barrier function.
