摘 要: 目的:研究手足口病(HFMD)并发脑炎患儿血清NSE、S100β含量与炎症反应、免疫应答的相关性。方法:选择在本院诊断为手足口病的患儿,将合并中枢神经系统的患儿作为重症组、未合并中枢神经系统的患儿作为轻症组;选择同期体检的儿童作为对照组。采集血清并检测NSE、S100β及炎症反应介质的含量,采集外周血并检测T细胞亚群、NK细胞的含量。结果:重症组和轻症组患者血清中NSE、S100β的含量均显著高于对照组,重症组患者血清中NSE、S100β的含量均显著高于轻症组;重症组和轻症组患者血清中TNF-α、IL-6、IL-10、IL-17、PCT的含量以及外周血中HLA-DR+CD4^+、HLADR+CD8^+、CD38^+CD4^+、CD38^+CD8^+的含量均显著高于对照组,外周血中CD3^+ T细胞、CD4^+ T细胞、CD8^+ T细胞以及NK细胞的含量均显著低于对照组;重症组患者血清中TNF-α、IL-6、IL-10、IL-17、PCT的含量以及外周血中HLA-DR+CD4^+、HLA-DR+CD8^+、CD38^+CD4^+、CD38^+CD8^+的含量均显著高于轻症组且与NSE、S100β含量呈正相关,外周血中CD3^+ T细胞、CD4^+ T细胞、CD8^+ T细胞以及NK细胞的含量均显著低于轻症组且与NSE、S100β含量呈负相关。结论:手足口病并发脑炎患儿血清NSE、S100β含量升高与炎症反应激活、免疫应答紊乱密切相关。 Objective:To study the correlation of serum NSE and S100βlevels with inflammatory response and immune response in children with hand-foot-mouth disease(HFMD)complicated by encephalitis.Methods:Children who were diagnosed with hand-foot-mouth disease in Yulin Third Hospital were selected,children who were complicated with central nervous system were selected as severe group,and children who had no complication of central nervous system were selected as mild group;children who received physical examination during the same period were selected as the control group.Serum was collected to determine the contents of NSE,S100βand inflammatory response mediators,and peripheral blood was collected to determine the contents of T cell subsets and NK cells.Results:Serum NSE and S100βlevels of severe group and mild group were significantly higher than those of control group,and serum NSE and S100βlevels of severe group were significantly higher than those of mild group;serum TNF-α,IL-6,IL-10,IL-17 and PCT levels as well as peripheral blood HLA-DR+CD4^+,HLADR+CD8^+,CD38^+CD4^+and CD38^+CD8^+levels of severe group and mild group were significantly higher than those of control group while peripheral blood CD3^+T cell,CD4^+ T cell,CD8^+T cell and NK cell levels were significantly lower than those of control group;serum TNF-α,IL-6,IL-10,IL-17 and PCT levels as well as peripheral blood HLA-DR+CD4^+,HLA-DR+CD8^+,CD38^+CD4^+and CD38^+CD8^+levels of severe group were significantly higher than those of mild group and positively correlated with NSE and S100βlevels while peripheral blood CD3^+T cell,CD4^+T cell,CD8^+T cell and NK cell levels were significantly lower than those of mild group and negatively correlated with NSE and S100βlevels.Conclusion:The increase of serum NSE and S100βlevels in children with HFMD complicated by encephalitis is closely related to inflammatory response activation and immune response disorder.
