摘　　要： 目的探讨经脐单孔腹腔镜疝囊高位结扎术治疗小儿腹股沟斜疝的临床效果。方法选取2014年4月~2016年4月我院收治的80例腹股沟斜疝患儿作为研究对象,按照简单随机化法将其分为传统开放组和腹腔镜组,每组各40例。传统开放组予以开放式疝囊高位结扎术,腹腔镜组予以经脐单孔腹腔镜疝囊高位结扎术。对比分析两组患者的各项手术指标、术后并发症发生率及疝复发情况。结果腹腔镜组患者的切口长度、手术时间及住院时间均短于传统开放组,出血量少于传统开放组,差异有统计学意义(P<0.05)。腹腔镜组术后并发症的总发生率(7.5%)远低于传统开放组(22.5%),差异有统计学意义(P<0.05)。手术1年后,腹腔镜组的疝复发率低于传统开放组,差异有统计学意义(P<0.05)。结论采用经脐单孔腹腔镜术治疗小儿腹股沟斜疝,具有切口小、创伤小、术中出血少、恢复快、住院时间短等特点,安全可靠,术后并发症发生率和疝复发率低,可在基层医院临床中广泛应用。 Objective To investigate the clinical effect of using the high ligation of hernia sac with single-pore transum- bilical laparoscopy to treat pediatric oblique inguinal hernia.Methods 80 cases of children with oblique inguinal hernia treated by our hospital from April 2014 to April 2016 were selected as the study subjects,they were divided into the traditional open group and the laparoscopy group according to the simple randomization method,there were 40 cases in each group.The traditional open group was given the open hernia sac high ligation,while the laparoscopic group was treated with high ligation of hernia sac with single-pore transumbilical laparoscopy.The surgical indexes,postoperative complications and hernia recurrence were compared and analyzed between the two groups.Results The incision length, the operation time and the length of stay in the laparoscopic group were shorter than traditional open group,the amount of blood was less than the traditional open group,differences were statistically significant （P〈0.05）.The total incidence of postoperative complications of laparoscopy （7.5%） was well below the traditional open group （22.5%）,and the difference was statistically significant （P〈0.05）. 1 year after operation,the rate of hernia recurrence in laparoscopic group was lower than that in traditional open group,and the difference was statistically significant （P〈0.05）.Conclusion Using single- pore transumbilical laparoscopy to treat the pediatric inguinal hernia has the characteristics of small incision,small trau- ma,less hemorrhage in the operation,rapid recovery and short hospital stay etc.This method is safe and reliable,and with lower incidence of postoperative complications and hernial recurrence,so it can be widely used in the clinic of basic- level hospital.
