曲霞可视喉镜在老年患者困难气道中的应用探讨

摘  要: 目的分析视曲霞斯美特视喉镜用于老年患者的困难气道的临床处理,在气管插管时的可行性与安全性进行探讨。方法从2015年9月至2016年6月手术患者中选择80例老年困难气道患者作为研究对象,在全身麻醉后随机分为两组,每组各40例,其中Q组使用曲霞斯美特可视喉镜插管,M组使用Macintosh直接喉镜进行插管。插管操作在两组患者中均由同一位主治医师以上职称、工作经验较为丰富的麻醉科医师完成.记录并分析诱导前、诱导时以及插管时、插管后心率、血压和血氧的变化情况,对比分析气管插管时间、成功率、并发症等情况。结果 Q组的插管的成功率明显优于M组,咽喉部黏膜没有明显的损害,血压、心率、血氧饱和度更为稳定。结论在老年患者的困难气道的处理中,曲霞斯美特可视喉镜比Macintosh喉镜更优势,操作过程中可清楚看见咽喉和气道的解剖结构,减轻了对咽喉部位的刺激和气管插管时的心血管反应。 Objective To analysis the clinical treatment of QuxiaSimeite Glidescope in elderly patients with difficult airway,and to investigate the feasibility and safety during endotracheal intubation.Methods Selected80cases elderly patients as the object of study with difficult airways from September2015to June2016,they were divided into two groups randomly after general anesthesia,there were40cases in each group.QuxiaSimeite glidescope were used in group B and Macintoshlaryngoscope were used in group M during endotracheal intubation.The all patients were performed by the same experienced anesthesiologist(specialist).The changes of heart rate,blood pressure and saturation were recorded and analyzed before and after induction,the time of intubation and after intubation.The length of intubation time,the success ratio and complications were analyzed.Results The success ratio of intubation in group B was significantly higher than that of in group M.There was no significantly mucosal damage in the patients'throat,blood pressure,heart rate and oxygen saturation are more stable.Conclusion Quxia glidescope is more effective in elderly patients with difficult airway than the McCoy laryngoscope,the anatomical structure of the throat and airway can be showed clearly,there is less stimulation on the throat and less cardiovascular response to the patients during the procedure.
