摘　　要： 目的:家庭医生制度下对社区COPD筛查与康复的有效管理模式。方法:我中心所在服务范围内的120例COPD患者,通过随机分组方式,将所有患者分为2组,干预组进行系列健康教育课程和群组中西医结合运动疗法训练,对照组给予目前常规的一般调查指导及诊疗,对两组患者管理结果加以对比。结果:干预组患者的行走距离、生活质量显著高于对照组,差异有显著性(P<0.05)。结论:家庭医生服务模式在提高COPD患者的生活质量和心理健康方面有积极作用。 Objective： An effective management model of community COPD screening and rehabilitation under family doctor System.Methods： 120 patients with COPD in the service area of Longhua street community service center of Shanghai Longhua street were divided into two groups by randomized grouping. The intervention group was followed by a series of health education courses and group Chinese and western medicine combined with exercise therapy. Given the current general general investigation guidance and diagnosis and treatment,the two groups of patients to compare the results of management. Results： The walking distance and quality of life of the patients in the intervention group were significantly higher than those in the control group（ P 0. 05）. Conclusion： The family doctor service model has a positive effect on improving the quality of life and mental health of COPD patients.
