学习交流制度对藏区援建医院医疗质量的影响

摘  要: 目的对比建立和落实学习交流制度前后藏区援建医院医疗质量指标,总结医疗质量改进经验。方法实施改进策略,包括:成立组织、建立制度及奖惩办法,医教科每周定期组织召开学习交流会,学习交流会上针对本周医院运行中存在的问题进行讲评和处理,开展医疗法规、医疗质量与安全、三基三严的学习或专题讲座。对比实施改进策略前后部分医疗质量指标。结果实施后,交接班制度执行率、手术审批制度执行率、疑难危重病例讨论制度执行率、死亡病例讨论执行率、患者医疗满意率、护理满意率、甲级病历合格率等高于实施前同期相关指标,实施后转院率、住院患者医院感染率均低于实施前同期相关指标,差异均有统计学意义;实施后的平均住院日与实施前差异无统计学意义;医、技、护执业资格通过率,医、技、护中级职称考试通过比例有所上升。结论因地制宜地建立和落实学习交流制度极大地提高了藏区援建医院的医疗质量,今后应坚持落实。 Objective To assess effect of the staff exchange on medical service quality of Tibetan hospitals and summarize relevant experience. Methods The staff exchange system included a reward and punishment system, routine learning and communicating about problems discovered last week, and lectures about medical security. We then compared the quality of medical service before and after the implementation of the staff exchange system. Results After the implementation of this system, the quality of medical service was significantly higher than before in many ways including the execution rate of handover, examination and approval of surgery, and dead cases discussion, patient satisfaction with medical service, proportion of A-class medical record. Meanwhile, the referral rate and the hospital infection rate was significantly lower than before. However, the average hospitalization days remained stable. In 2016, the passing rate of medical, technical, and nursing practice qualification increased than in 2015. Conclusion The establishment of the staff exchange system according to local conditions can greatly improve the medical service quality.
