摘 要: 目的探究血清白蛋白(ALB)水平与高血压性脑出血血肿周围水肿的相关性。方法 2012年2月-2016年2月本院收治的230例高血压性脑出血患者为研究对象,依据患者血清ALB水平分为观察组(ALB≤35 g/L)与对照组(ALB>35 g/L),再将观察组分为重度组(<25 g/L)、中度组(25 g/L^30 g/L)和轻度组(30 g/L^35 g/L)。分别在入院时、入院后1 d、4 d和14 d分析不同时期的相对水肿体积指数(REI)和水肿体积变化指数(AEI),并分析血清ALB与其的相关性。结果入院后1 d时2组患者REI、AEI相比,差异均无统计学意义(P>0.05);入院后4 d和14 d时,观察组的REI、AEI均高于对照组,差异有统计学意义(P<0.05)。随着观察组病情的加重,患者的REI不断增加,差异有统计学意义(P<0.05)。AEI与血清ALB水平呈负相关(r=-0.738,P<0.05)。结论血清ALB水平过低会导致高血压脑出血患者的水肿程度加重,且其严重程度与血清ALB的水平呈负相关。 Objective To investigate the relationship between serum albumin( ALB) level and hematoma edema in patients with hypertensive intracerebral hemorrhage( ICH). Methods 230 cases of hypertensive cerebral hemorrhage patients hospitalized in this hospital from February 2012 to February 2016 were selected as the research objects. They were randomly divided into observation group( ALB = 35 g/L) and control group( ALB 35 g/L) on the basis of the level of serum ALB; the observation group was divided into severe group( 25 g/L),moderate group( 25 g/L-30 g/L) and mild group( 30 g/L-35 g/L). The relative edema volume index( REI) and the volume change index( AEI) of different periods were analyzed at 1 d,4 d and 14 d after admitted in hospital,respectively and the correlation with ALB was analyzed. Results REI and AEI in the two groups were compared after admission 1 d,and the difference was not statistically significant( P〈0. 05); after admission 4 d and 14 d,REI and AEI of the observation group were higher than those of the control group,and the difference was statistically significant( P〈0. 05). With the aggravation of the observation group,the REI of the patients increased,and the difference was statistically significant( P〈0. 05). AEI was negatively correlated with serum ALB( r =-0. 738,P〈0. 05). Conclusion The excessive low level of serum ALB can lead to the degree of edema in patients with hypertensive cerebral hemorrhage,and the severity is negatively correlated with the ALB level of serum.
