钙卫蛋白和基质细胞衍生因子-1α在恶性胸腔积液中的表达水平及临床意义

摘  要: 目的探讨钙卫蛋白和CXCL12在恶性胸腔积液中的表达水平及其临床意义。方法选取胸腔积液患者105例作为研究对象,包括恶性胸腔积液患者50例和非恶性胸腔积液患者55例,采用ELISA法测定胸腔积液中钙卫蛋白和CXCL12水平。结果恶性胸腔积液组患者的钙卫蛋白和CXCL12水平明显低于非恶性胸腔积液组,差异均有统计学意义(P<0.05)。ROC曲线分析,胸腔积液钙卫蛋白曲线下面积(AUC)为0.81,灵敏度为71.2%,特异度为81.7%;胸腔积液CXCL12 AUC为0.78,灵敏度为70.5%,特异度为76.4%。结论钙卫蛋白和CXCL12的检测对恶性胸腔积液与非恶性胸腔积液的鉴别诊断有一定的参考价值。 Objective To investigate the expression levels and clinical significance of calprotectin and stromal cell derived factor-1( CXCL12) in malignant pleural effusion. Methods 105 cases of patients with pleural effusion were selected as the research object,including 50 cases of malignant pleural effusion and 55 cases of non-malignant pleural effusion,the levels of calprotectin and CXCL12 in pleural effusion were detected by enzyme-linked immunosorbent assay( ELISA). Results The levels of calprotectin and CXCL12 in malignant pleural effusions group were obviously lower than that of non-malignant pleural effusion group,and the differences were statistically significant( P〈0. 05). ROC curve analysis showed that the surface area( AUC) was 0. 81,the sensitivity was 71. 2%,and the sensitivity was 81. 7%. AUC of chest fluid CXCL12 was 0. 78,sensitivity was 70. 5%,and the specificity was 76. 4%. Conclusion Detection of calprotectin and CXCL12 has a certain value in the differential diagnosis of malignant pleural effusion and non-malignant pleural effusion.
