摘　　要： 目的:探究空心钉减压联合介入治疗对早期股骨头无菌性坏死疗效及对预后的影响。方法:选择我院内股骨头无菌性坏死的早期患者65例,患者被随机划为实验组及对照组。实验组34例予空心钉减压联合介入治疗;对照组31例予介入治疗。在手术结束后6个月,分别对两组患者进行回访调查,内容包括评价患者髋关节功能的恢复情况,疼痛是否好转,血流是否改善及生活质量评分等指标,分布进行对比及分析。结果:实验组总有效率97.06%高于对照组77.42%,差异存在统计学意义(χ~2=5.795,P<0.05);实验组患者股骨头处血流改善情况为88.24%高于对照组41.94%,差异存在统计学意义(χ~2=16.277,P<0.05);治疗后,两组患者Harris评分与下肢运动功能评分均增加,与对照组相对比,实验组Harris评分与下肢运动功能评分较高,差异有统计学意义(P<0.05);治疗后实验组生活质量评分较高,差异具有统计学意义(P<0.05)。结论:对早期股骨头无菌性坏死患者进行治疗时,空心钉减压联合介入治疗,可以使股骨头处血运得到有效改善,增强股骨头负重区的支撑作用,改善关节的功能作用,适合推广应用。 Objective： To investigate the efficacy of cannulated screws combined with interventional therapy in the treatment of aseptic necrosis of femoral head. Method： 65 early aseptic necrosis of femoral head in our hospital were selected and randomly divided into the control group and the experiment group.34 cases in the experimental group was treated with cannulated screws combined with interventional therapy; 31 cases in the control group were treated with interventional therapy. 6 months after surgery,two groups of patients were using questionnaires,including the evaluation of patients with hip joint function recovery,the pain is better,and improve the quality of life score indexes such as blood flow distribution were compared and analysis weather. Results： The total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant（ χ^2= 5.795,P〈0.05）. The improvement of blood flow in the femoral head of the experimental group was higher than that of the control group,and the difference was statistically significant（ χ^2= 16.277,P〈0.05）.After treatment,the two groups of patients with Harris score and limb motor function score were increased. Compared with the control group,the experimental groupHarris score and limb motor function score was higher,the difference was statistically significant（ P〈0. 05）; After treatment,the quality of life score of the experimental group was higher,the difference was statistically significant（ P〈0.05）. Conclusion： The early avascular necrosis of the femoral head were treated with cannulated screw,decompression combined with interventional therapy,can make the femoral head blood supply has been effectively improved,and increase the support function of the weight-bearing area of the femoral head,improve joint function,suitable for application.
