肽基精氨酸脱亚氨酶4在类风湿关节炎中的研究进展

摘  要: 类风湿关节炎(RA)是一种常见的自身免疫性疾病,其发病机制复杂,多种细胞因子在RA患者血清中均高表达,同时RA患者血清中也会出现多种自身抗体,如类风湿因子、抗环瓜氨酸肽抗体(CCP)和抗角蛋白抗体等,但仍有部分RA患者这些抗体为阴性。肽基精氨酸脱亚氨酶4(PAD4)是一种酶,在多种细胞中均表达,其在RA中异常表达,诊断RA的特异性较高,对RA的诊断和治疗有重要作用,是RA新型的血清学指标;PAD4不仅在表达水平上对RA有诊断价值,而且单核苷酸多态性也与RA发病、抗CCP抗体水平以及影像学的严重程度密切相关。 Rheumatoid arthritis (RA) is a common autoimmune disease, and the pathogenesis is complex. The expression of a variety of cytokines are higher in the serum of patients with RA, and many autoantibodies are detected in in the serum of RA patients, such as rheumatoid factor and anti-cyclic citrullinated peptide antibody(CCP) and anti-keratin antibody and so on, but these antibodies are negative in the serum of RA patients. The peptidyl arginine deiminase 4 (PAIM) is an enzyme which is expressed in a variety of cells, and is abnormally expressed in rheumatoid arthritis. PAD4 has high specificity for the diagnosis of RA, and it plays an important role in the diagnosis and treatment of rheumatoid arthritis, thus is a new serological biomarker of RA. The PAIM is very important for the diagnosis of RA not only on expression level, but also on single nucleotide polymorphism,which are closely related to the pathogenesis of RA, the level of anti-CCP antibody and the imaging examination indicated severity.
