摘　　要： 目的探讨基质金属蛋白酶-14(MMP-14)、MMP-2及基质金属蛋白酶组织抑制因子-2(TIMP-2)在胃癌、癌旁组织及胃溃疡中的表达情况及意义。方法选取2014年5月至2016年6月该院收治的确诊为胃癌及胃溃疡的患者各30例作为研究对象,采用免疫组织化学方法检测患者MMP-14、MMP-2、TIMP-2的表达情况,并分析其相互关系。结果胃癌组织中MMP-14及MMP-2的阳性表达情况均比癌旁组织及胃溃疡组织中阳性表达高(P<0.05);胃癌组织中TIMP-2的阳性表达例数比癌旁组织及胃溃疡组织中阳性表达低(P<0.05);MMP-2阳性表达情况与MMP-14呈正相关(r=0.15,P>0.05);TIMP-2表达情况与MMP-14、MMP-2呈负相关(r=-0.21、-0.36;P<0.05);MMP-14、MMP-2及TIMP-2阳性表达率与胃癌浸润深度、淋巴结转移及TNM分期均存在相关性(P<0.05);阳性表达率仅与淋巴结转移有关(P<0.05)。结论TIMP-2与MMP-2存在抑制和被抑制的关系,TIMP-2与MMP-14也可能存在对抗作用,三者表达失衡可能是胃癌转移的原因,可作为胃癌恶性程度的重要预测因子。 Objective To investigate the expression and significance of matrix metalloproteinase-14（MMP-14）,MMP-2and tissue inhibitor of metalloproteinase-2（TIMP-2） in gastric carcinoma,paracancerous tissues and gastric ulcer. Methods Each 30 cases diagnosed as gastric cancer and gastric ulcer in our hospital from May 2014 to June 2016 were selected as the study subjects. The expression levels of MMP-14,MMP-2 and TIMP-2 were detected by adopting the immunohistochemistrical method and their interrelationships were analyzed. Results The positive expression rates of MMP-14 and MMP-2 in gastric cancer tissues were higher than those in paracancerous tissues and gastric ulcer tissues（P〈0.05）. The case number of TIMP-2 positive expression in gastric cancer tissues was lower than that in paracancerous tissues and gastric ulcer tissues（P〈0.05）. The positive expression of MMP-2 was positively correlated with MMP-14 r =0.15,P〈0.05）;the expression of TIMP-2 was negatively correlated with MMP-14 and MMP-2（（r=-0.21,-0.36,P〈0.05）. The positive expression rates of MMP-14,MMP-2 and TIMP-2 were correlated with the depth of invasion,lymph node metastasis and TNM staging（P〈0.05）. The TIMP-2 positive expression rate was only related to lymph node metastasis（P〈0.05）. Conclusion The inhibition and inhibited relationship exist between TIMP-2 and MMP-2,and there may also be antagonism relation between TIMP-2 and MMP-14. The expression unbalance of these three indicators may be the cause of gastric cancer metastasis,which can be used as an important predictor of gastric cancer malignancy degree.
