摘 要: 目的探讨慢性阻塞性肺疾病(COPD)患者多排螺旋CT(MSCT)肺测量指标与肺功能检查(PFT)指标的相关性。方法选择2015年6月至2016年3月因COPD入成都医学院第一附属医院治疗,且同时行MSCT和PFT检查CT容积重建成功的患者共70例;双肺各选择上、中、下3个层面点值法测量肺CT值,再利用后处理软件分析全肺容积及肺气肿率,行肺CT值、肺容积、肺气肿率与PFT检查结果的相关性。结果双肺上、中、下3个层面肺CT值与FEV1%具有较强的相关性(P<0.05),右侧、左中层面CT值与RV/TLC弱相关(P<0.05),全肺平均CT值与FEV1%、FEV1/FVC和RV/TLC具有较强的相关性(P<0.05)。CT测量肺总容积与TLC、MVC、RV和RV/TLC弱相关(P<0.05)。CT测量肺气肿率与PFT各指标均有相关性(P<0.05),其中,与RV/TLC、FEV1%的相关性较强(rs>0.5)。结论 MSCT肺部扫描CT值、肺容积及肺气肿率等测量值与PFT部分指标具有相关性,具有辅助评估COPD患者肺通气功能的应用价值。 Objective To investigate the correlation between pulmonary quantitative indicators of multi slice spiral CT (MSCT) and pulmonary function tests (PFT)in chronic obstructive pulmonary disease (COPD). Methods 70 COPD patients hospitalized in The First Affiliated Hospital of Chengdu Medical College from June of 2015 to March of 2016 and having undergone MSCT scan and PFT successfully were selected in the study. The CT values of the upper, middle and lower levels of the right and left lungs were measured and the lung density software was used to analyze the total pulmonary volume and the emphysema rate. Then the statistical analysis was applied to analyze the correlation of the PFT results with the CT values, pulmonary volume, and emphysema rate respectively. Results The CT values of the upper, middle and lower levels of both lungs were strongly correlated with FEV1% respectively (P〈0. 05), while The CT values of the right lung and middle level of the left lung were weakly correlated with RV/TLC respectively (P〈0.05). The mean CT value of the whole lung was strongly correlated with FEV1%, FEV1/FVC and RV/TLC respectively (P〈0. 05). The emphysema rate measured by CT was correlated with each index of PFT respectively (P〈 0. 05), and it was strongly correlated with RV/TLC and FEV1% respectively (rs〉0.5). Conclusion The CT values such as the values of CT density, lung volume and emphysema rate measured by MSCT are correlated with some indexes of PFT, so they have some application value of helping assess the pulmonary ventilation function in COPD patients.
