摘　　要： 目的:研究益肾降浊方对慢性肾炎患者钙磷代谢及甲状腺激素代谢紊乱的影响机制。方法:选取我院2014年12月至2016年12月慢性肾炎患者104例,按入院顺序编号,采取随机数表法将患者分为观察组与对照组两组,每组52例,给予对照组常规西药治疗,观察组在对照组治疗基础上联合益肾降浊方治疗,比较两组临床疗效以及治疗前后肾功能、钙磷代谢、甲状腺激素代谢指标水平变化情况。结果:观察组患者临床治疗总有效率为90.38%较对照组67.31%显著较高(P<0.05);治疗后观察组血清肌酐(Scr)、血尿素氮(BUN)水平较对照组显著较低,肌酐清除率(Ccr)水平较对照组显著较高(P<0.05);观察组血磷、钙磷乘积水平较对照组显著较低,观察组血钙水平较对照组显著较高(P<0.05);观察组三碘甲状腺原氨酸(T_3)、总甲状腺素(T_4)水平较对照组显著较高,促甲状腺素(TSH)水平较对照组显著较低(P<0.05)。结论:益肾降浊方联合常规西药治疗能有效改善慢性肾炎患者临床症状,调节钙磷代谢及甲状腺激素紊乱状态,改善肾功能,安全可靠,值得临床应用推广。 Objective： The influence mechanism of Yishen Jiangzhuo Recipe on hormone metabolism of calcium and phosphorus in patients with chronic nephritis and thyroid metabolism. Methods： In our hospital from December 2014 to December 2016,104 cases of patients with chronic nephritis,numbered according to the order of admission,were randomly divided into observation group and controlled group,each group had 52 cases,the controlled group received conventional western medicine treatment,the observation group was given Yishen Jiangzhuo Recipe on the basis of the controlled group,clinical effects of the two groups before and after treatment as well as renal function,calcium and phosphorus metabolism,metabolism of thyroid hormone levels were compared. Results： In the observation group,the total efficiency of clinical treatment（ 90. 38%） was significantly higher than the controlled group（ 67. 31%）（ P〈0. 05）; Serum creatinine（ Scr）,blood urea nitrogen（ BUN） levels of the observation group after treatment were significantly lower than the controlled group,creatinine clearance rate（ Ccr） level was significantly higher than the controlled group（ P〈0. 05）; Blood phosphorus,calcium and phosphorus product level of the observation group were significantly lower than the controlled group,serum calcium level of the observation group was significantly higher than the controlled group（ P〈0. 05）; Three triiodothyronine（ T_3）,total thyroxine（ T_4） levels of the observation group were significantly higher than the controlled group,thyroid stimulating hormone（ TSH） level of the observation group was significantly lower than the controlled group（ P〈0. 05）. Conclusion： Yishen Jiangzhuo Decoction combined with conventional western medicine therapy can effectively improve the clinical symptoms of patients with chronic nephritis,regulating calcium phosphorus metabolism and thyroid hormone disorder,improve the renal function,is safe and reliable,worthy of clinical application.
