摘　　要： 目的:研究人性化护理对剖宫产后瘢痕子宫产妇自然分娩的临床效果。方法:选择2015年2月至2016年11月进行分娩的剖宫产后瘢痕子宫患者174例,随机分为观察组与对照组,每组各87例。对照组采用常规护理干预,观察组采用人性化护理干预,比较两组产妇的分娩方式,泌乳时间,抑郁自评量表评分以及焦虑自评量表评分。结果:观察组产妇的阴道顺产率高于对照组(χ~2=16.732,P=0.000),阴道助产率和剖宫产率低于对照组(χ~2助产=9.439,P=0.001;χ~2剖宫产=13.234,P=0.000);观察组产妇的乳汁分泌时间<1 d和≥1 d且<2 d的比例高于对照组(χ~2<1 d=11.235,P=0.000;χ~2≥1 d=9.387,P=0.002),乳汁分泌时间≥2 d且<3 d、≥3 d和泌乳困难的比例低于对照组(χ~2≥2 d且<3 d=16.436,P=0.000;χ~2≥3 d=17.238,P=0.000;χ~2分泌困难=12.457,P=0.000);观察组产妇的抑郁自评量表评分以及焦虑自评量表评分均低于对照组(t抑郁=16.534,P=0.000;t焦虑=19.739,P=0.000)。结论:人性化护理干预可以有效促进剖宫产后瘢痕子宫产妇的自然分娩,促进其泌乳,并改善产妇的焦虑情绪。 Objective： To study the clinical effect of humanized nursing intervention on promoting natural delivery of scarred uterus after cesarean section. Methods： 174 patients with scarred uterus after cesarean section from February 2015 to November 2016 were randomly divided into the observation group and the control group,87 cases in each group. The control group was treated with routine nursing intervention while the observation group was treated with humanized nursing intervention,with the mode of delivery,the lactation time,the self-rating depression scale and the anxiety self-rating scale compared. Results： The vaginal delivery rate in the observation group was significantly higher than that in the control group（ P〈0. 05）. The forceps-assisted vaginal delivery rate and the cesarean section rate in the observation group were significantly lower than those in the control group（ P〈0. 05）. The incidence rate of lactation time 1 d and≥1d but 2 d in the observation group was significantly higher than that in the control group（ P〈0. 05）; The incidence rate of lactation time≥2d but 〈3 d and 〉4 d and the ratio of lactation difficulty in the observation group were significantly lower than those in the control group（ P〈0. 05）. The scores of the self-rating depression scale and the anxiety self-rating scale in the observation group were significantly lower than those in the control group（ P〈0. 05）. Conclusion： Humanized nursing intervention can effectively promote the natural delivery of the scarred uterus after cesarean section,promote maternal lactation and improve maternal anxiety.
