摘 要: 目的探索对乙酰氨基酚(APAP)致肝损伤的研究现状及热点。方法以医学主题词表(Me SH)制定检索策略,检索Pub Med/MEDLINE数据库,获取对乙酰氨基酚致肝损伤的相关文献,将题录信息导入Thomson Data Analyzer(TDA)软件,对文献基本特征进行解析,用可视化网络图展示作者间关系,并采用共词及聚类分析方法对高频Me SH词进行热点分析。结果检索入选文章1 714篇,1966~2015年发文数量随年份呈增长趋势;美国和英国刊发文章数量最多;《肝病学》(Hepatology)杂志刊登的文章数量最多,《柳叶刀》(Lancet)影响因子最高;药物过量和药物间的相互作用是肝脏损伤的主要诱因;丙氨酸转氨酶(ALT)、谷胱甘肽(GSH)和天门冬氨酸氨基转移酶(AST)是重要的肝损伤检测指标;氧化应激和脂质过氧化是最常见的发生机制,毒性结局更多关注的是急性肝衰竭。结论通过定量文献分析,确定对乙酰氨基酚致肝损伤的重要机制,植物疗法(植物提取物)是肝脏保护的前沿研究,以及确定临床治疗方法和流行病学调查的热点;为今后进一步开展损伤机制研究,药物安全性评价和临床合理用药提供宏观信息参考。 Objective To explore the field of acetaminophen-induced liver injury,including the research status and hot themes,through the method of bibliometric analysis. Methods The articles were selected by searching in Pub Med/MEDLINE using Medical Subject Headings( Me SH) terms and processed by the software of Thomson Data Analyzer( TDA). Basic features of literature were summarized.Analysis on collaborative researchers and hot study themes were conducted using co-word cluster analysis via high-frequency Me SH Terms.Results A total of 1714 articles were retrieved. The number of published articles was increasing gradually from 1966 to 2015. The countries from the United States and England conducted most of the articles. Hepatology published the largest number of journals and Lancet showed great influence in this field. Drug overdose and drug interactions were the incentives. Alanine aminotransferase,glutathione,aspartate aminotransferase were the fatal detection indexes. Oxidative stress and lipid peroxidation were the most common mechanisms. Acute liver failure was the main outcome of the special concern. Conclusion Based on the quantitative analysis of bibliographic information,the current hot themes were defined,including mechanism of acetaminophen-induced liver injury,hepatic encephalopathy specifically the plant extract treating liver injury,clinical treatment and epidemiological investigation. It would provide comprehensive reference for researchers in this field of study and its application on mechanism research,assessment of safety and rational clinical use of drugs.
