摘 要: 目的运用品管圈管理方法分析我院救护车转运风险致意外伤害的发生原因,并针对真因(工具因素)拟定改进措施,从而降低救护车意外伤害事件的发生,降低意外伤害事件的原因发生率。方法严格按照品管圈程序进行品管圈活动,随机抽取我院2014年至2015年500次出诊记录,分析意外伤害事件分类和发生原因。在意外伤害事件的高发点,针对真因采取担架仓挡板改良措施和辅助措施。确认改进效果,使成果固化,形成科室保证医疗服务质量的标准化制度。结果经过担架仓挡板的改良和辅助措施的实施,意外伤害的原因发生率从14.57%下降4.67%,差异有统计学意义(P<0.05)。同时救护车担架仓挡板的改良获得国家专利,专利号:201620104400.1,并形成产品,在我院54家双向转诊医院推广应用。结论担架仓挡板的改良,扩宽了支撑面,提高了稳定性,增加了承重力,以低成本的改良降低了救护车转运风险致患者意外伤害的发生率,减少了医疗纠纷,避免了高额赔付。 Objective To analyze the reasons of accidental injury caused by ambulance transport risk in our hospital by using QCC management method, and to make improvement measures according to real cause (tool factor), so as to reduce the occurrence of ambulance accidental injury events and reduce the cause occurrence of accidental injury events. Methods The quality control circle activities were carried out strictly according to quality control procedures. Records of 500 visits from 2014 to 2015 in our hospital were randomly selected, and the classification and causes of unintentional injuries were analyzed. At the high occurrence- point of the accidental injury, the improved measures and auxiliary measuresof thestretcher baffle according to the real cause were taken. Andwe confirmed the effect of improvement ,solidified the result and formed a standardized system to ensure the quality of medical serv- ice. Results After the improvement of the stretcher baffle and the implementation of auxiliary measures, the incidence of unintentional injury decreased by 4. 67% from 14. 57% ,and the difference was statistically significant(P 〈 0. 05 ). At the same time ,the improvement of thestretcher baffle of ambulance was granted national patent, patent number :201620104400. 1. And products were formed,which were popularized and applied in 54 two-way referral hospitals in our hospital. Conclusion The modified stretcher bafflewidens the support surface, improves the stability, and increases the bearing capacity ,which reduces the incidence of accidental injury in patients caused by risk of ambulance transport ,reduces medical disputes, and avoids high payment with low costs of modification.
