摘　　要： 目的分析无创通气在ICU治疗COPD合并重度CO_2潴留的疗效。方法选择我院ICU自2014年1月—2017年6月收治的65例COPD合并重度CO_2潴留患者作为研究对象,均给予无创通气操作,分析其具体效果。结果治疗前,患者的呼吸频率(29.03±2.92)次/min、心率(118.39±15.29)次/min及PaCO_2(97.38±11.29)mm Hg明显较正常水平高,p H值(7.21±0.02)、PaO_2(69.93±7.28)mm Hg较正常明显低;2 d后恢复正常,且与治疗前对比,各指标均明显改善,差异具有统计学意义(P<0.05);PaCO_2水平虽没有降至正常范围,但较治疗前对比,降低幅度较大,患者的症状好转,,差异具有统计学意义(P<0.05)。结论要有效的将COPD合并重度CO_2潴留者PaCO_2水平明显降低,改善其临床症状,可优先考虑采用无创通气。 Objective To analyze the efficacy of noninvasive ventilation in the treatment of COPD complicated with severe CO2 retention with 1CU. Methods 65 cases of COPD with severe CO2 retention in our hospital from January 2014 to June 2017 were selected as the study subjects in ICU, noninvasive ventilation was performed, its specific effect was selected. Results Before treatment, the respiratory rate （29.03 ~ 2.92） times/rain, heart rate （118.39 ：k 15.29） times/min and PaCO2 （97.38 ：k 11.29） mmHg were significantly higher than normal, and the value of pH （7.21 ± 0.02）, PaO2 （69.93±7.28） mmHg was significantly lower than normal range. 2 days returned to normal, and compared with before treatment, all indicators were significantly improved, the difference was statistically significant （P〈0.05）. The level of PaCO2 did not fall below normal range, but compared with before treatment, the decrease was greater, the symptoms of patients improved, and the difference was statistically significant （P〈0.05）. Conelusion To effectively COPD combined with severe CO2 retention PaCO2 value was significantly reduced to improve their clinical symptoms, can be given priority to the use of noninvasive ventilation.
