摘　　要： 目的探讨老年高血压餐后低血压患者的血压变异性规律。方法选取住院老年原发性高血压患者114例,行24h动态血压监测,分为餐后低血压组(PPH)72例与非餐后低血压组(NPPH)42例,比较两组一般资料及两组动态血压指标。用多因素线性回归分析来确定PPH独立危险因素。结果入选的老年原发性高血压患者PPH检出率为63%,其中早餐后32.5%午餐后30.7%晚餐后22.8%;PPH组24h、白天及夜间平均收缩压均显著高于NPPH组(P<0.05);PPH组血压变异性指标中24h收缩压变异系数、白天收缩压变异系数、夜间收缩压变异系数、夜间舒张压变异系数,明显高于NPPH组(P<0.05);老年高血压PPH检出率与年龄、高血压持续时间、糖尿病发生率、餐前基础血压值成正相关(P<0.05)。结论老年高血压PPH患者血压波动较明显以收缩压为著,且血压变异性大,年龄、高血压持续时间、糖尿病发生率、餐前基础血压值是老年原发性高血压PPH的独立危险因子,应加强重视与干预。 Obj e ctive to investigate blood pressure variability of elderly hypertension patients with postprandial hypotension. Methods choose 114 cases hospitalized elderly patients with primary hypertension, treat them with 24h ambulatory blood pressure monitoring, divide them into postprandial hypotension group （PPH） 72 cases and non postprandial hypotension group （NPPH） 42 cases, compare general data and dynamic blood pressure index of two groups. Identify PPH independent risk factors by multivariate linear regression analysis. Results PPH detection rate of elderly primary hypertension patients was 63%, including 32.5% after breakfast and 30.7% after lunch, 22.8% after supper; average systolic blood pressure of 24h, day and night time of PPH group was significantly higher than NPPH group （P 〈 0.05）; among blood pressure variability indexes of PPH group, 24h systolic blood pressure variation coefficient, day and nighttime systolic blood pressure variation coefficient, nocturnal diastolic blood pressure variation coefficient were significantly higher than NPPH group （P〈 0.05）; and PPH detection rate of elderly hypertension patients is positive con＇elated with age, duration of hypertension, diabetes incidence rate, pre-prandial basic blood pressure （P〈0.05）. Conclusion blood pressure fluctuation of elderly hypertension PPH patients showed systolic blood pressure obviously with blood pressure variability, PPH independent risk factors of elderly patients with primary hypertension include age, hypertension duration, diabetes incidence rate, pre-prandial basic blood pressure, it is necessary to pay attention and strengthen intervention.
