摘　　要： 目的探讨双心护理在高龄冠状动脉粥样硬化性心脏病(冠心病)伴焦虑抑郁情绪患者中的临床应用效果。方法连续入选2014年5月~2016年7月于陆军总医院干四科收治的高龄冠心病合并焦虑抑郁症状患者104例,采用随机分组的方法将患者分为观察组和对照组,其中,对照组(52例)实施常规护理,观察组(52例)在常规护理的基础上实施双心护理。借助焦虑自评量表(SAS)和抑郁自评量表(SDS)对两组患者实施相应护理前后的心理状态进行评估,借助超声心动图对其心功能进行评估,并对其遵医依从性和护理满意度进行评价。结果实施护理前,两组患者的超声心动图指标、SAS评分、SDS评分比较差异无统计学意义(P>0.05);经相应护理后,两组患者的心功能均有所改善,观察组超声心动图指标优于对照组,SAS、SDS评分均低于对照组,差异有统计学意义(P均<0.05)。遵医依从性观察组优于对照组(96.15%vs.76.92%);护理满意度观察组高于对照组(98.08%vs.82.69%),差异有统计学意义(P均<0.05)。结论双心护理能够有效改善高龄冠心病伴焦虑抑郁患者不良心理情绪,提高其遵医依从性,从而提升临床治疗效果和护理效果,提高护理满意度,利于心功能恢复,值得临床推广应用。 Objective To investigate the clinical application effect of psychocardial nursing service in elderly patients with coronary heart disease(CHD)complicated by anxiety and depression.Methods The patients(n=104)were continuously chosen from the Fourth Department of Geriatrics of Chinese PLA Army General Hospital from May2014to July2016,and then divided randomly into control group(n=52)received routine nursing service and observation group(n=52)received routine and psychocardial nursing service.The psychological states were reviewed by using self-rating anxiety scale(SAS)and self-rating depressive scale(SDS)in2groups before and after nursing intervention.The heart function was detected by using echocardiogram,and medical compliance and degree of satisfaction to nursing service were reviewed in2groups.Results The comparisons in indexes of echocardiogram,SAS scores and SDS scores had no statistical significance between2groups before the nursing service(P>0.05).After corresponding nursing service,the heart function was improved in2groups.The indexes of echocardiogram were better,and SAS scores and SDS scores were lower in observation group compared with control group(all P<0.05).The medical compliance was higher in observation group than that in control group(96.15%vs.76.92%),and degree of satisfaction to nursing service was higher in observation group than that in control group(98.08%vs.82.69%,all P<0.05).Conclusion The psychocardial nursing service can effectively ameliorate psychological negative emotions,promote medical compliance,and improve clinical efficacy,nursing efficacy and degree of satisfaction to nursing service.It is well worth clinical application.
